PREVENTIVE HEALTH ALLIANCE PROGRAMS
Clear the AirSM is the step you take BEFORE deciding to quit tobacco for good. The idea is simple: have a
good plan before you quit. Clear the Air empowers you to identify your reasons for quitting tobacco, helps you
develop a plan to quit for good and prepares you for the challenging task of life-long tobacco cessation. You
will receive coupons for nicotine replacement therapy.

Diabetes Awareness and PreventionSM is a 4-week program that focuses on the prevention and
management of diabetes, whether individuals are maintaining good health, at risk for, diagnosed with or care
about someone who is at risk for diabetes.

bone health and prevent or manage osteoporosis. You’ll learn how to establish a comprehensive bone-building

Personal Nutrition CoachingSM is a one-on-one counseling service with a Registered Dietitian
designed to help you make healthy lifestyle and nutrition choices for better health or to address specific
concerns, such as weight management, heart health and diabetes. Record your food intake and physical
activity with your own Lifestyle Log and discover the power of better nutrition. Available for adults and children.
