GP News Edition Page 1 of 4 Edition 13 - April 2013 Staff Briefings about Francis Report We held a number of staff briefings about the Francis Report which all staff were invited to attend. Over 1,000 staff attended. These sessions were mainly led by the Chief Executive, Andy Hardy. These briefings were for staff to get an understanding about what the Francis Report sets out and about UHCW's plans for the future. We received positive feedback from these briefings which will help the Trust shape its future plans. Additionally I would like to inform you that the Trust's Chairman, Phil Townshend will step down from his role in May 2013 at UHCW. I would like to take this opportunity to wish him all the best in his future pursuits and thank him for his tremendous contribution to UHCW during his time here. Notice about GP letters sent to UHCW UHCW would like to politely ask that all Meghana Pandit Chief Medical Officer Changes to Dementia Risk Assessment GPs will now receive referral letters for inpatients with a positive Dementia Risk Assessment or a diagnosis of delirium. As the Trust is working towards a paperless environment, these letters will only be sent electronically to each GP’s Docman system. GPs without Docman will continue to receive paper copies for the time being. GPs letters that are written for patients that are coming into hospital to be marked DEAR MEDICAL TEAM, DEAR SURGICAL TEAM etc. This is to ensure the flow of patients are dealt with correctly, especially as the Trust has implemented a new model of working between Emergency Medicine and Acute Medicine / Medicine which went live in February 2013. Good Transfusion Sampling Guide In order to prevent serious blood transfusion incidents due to mis-identification of samples, Blood Bank at UHCW will reject samples if the following national guidance is not strictly adhered to: The Transfusion Request Form MUST contain the following patient identification details (banda label/addressograph labels are acceptable on request forms): Surname (correctly spelt) Forename (in full and correctly spelt) Date of birth (not age or year of birth) Hospital number (NHS number acceptable only for antenatal/CRM samples) Gender (and/or on the specimen) Collector’s details Name/Signature of requesting medical officer The Sample MUST have the following details handwritten on it: (Banda label/addressograph labels are NOT acceptable on sample bottles): Paula Dodgson, GP Liaison Nurse can be contacted on 02476966223 or you can email [email protected] or [email protected]. Update on UHCW consultants Since February 2013 to April 2013 the Surname (correctly spelt) Forename (in full and correctly spelt) Date of birth (not age or year of birth) Hospital number (NHS number acceptable only for antenatal/CRM samples) Gender (and/or on the request form) Collector’s details Date and Time specimen collected There will be no exceptions to this, and when a sample is rejected the patient will have to be bled again. A new sample will have to be sent to Blood Bank with a new form. Currently 42% of all rejections are because the collector’s details are either not on the sample or the Transfusion Request Form. Abdominal Aortic Aneurysm (AAA) Screening and Surveillance Programme, Coventry and Warwickshire The local Abdominal Aortic Aneurysm (AAA) Screening & Surveillance Programme for Coventry and Warwickshire has been in place for the past year. This is part of a NHS national screening programme which is being implemented gradually across England, and will cover the whole of England by April 2013. following consultants have started at the Trust: Mr Nonavinakere Manjunatha, Opthalmology Mr Zaed Zoher Raouf, Consultant Colorectal Surgeon Mr Gabriele Marangani, Consultant Colorectal Surgeon Since February 2013- April 2013, the following five consultants have left the Trust: Dr Kamran Rostami, Gastroenterology; Dr Arun Dheer, Radiology; Ms Rabia Din, Neuro Rehab; Mr Alan Duby, Trauma and Orthopaedics; Dr Hatim Ali, Care of the Elderly and Acute Medicine; Dr Edwin Borman, Anaesthetics and Dr Krishnendu Ghosh, Gerontology. The Arden Local Area Team (LAT) commissions UHCW to provide this screening programme for the local area. file://\\webdev2\intranet_communication\docuements\GP News\2013\GP News editio... 04/07/2013 GP News Edition Page 3 of 4 The programme screens the cohort of men who turn 65 between 1st April 2013 to 31st March 2014 in Coventry and Warwickshire – approx. 5,000 men. If men over 65 would like to be screened they can self refer to the programme directly. This screening programme aims to be accessible to patients and to deliver healthcare in the community. In Coventry, screening takes place in the City of Coventry Health Centre and screening is available at most Warwickshire GP practices over the screening year. It is important for GPs to encourage patients to attend this quick and free scan to reduce their chances of suffering a burst aortic aneurysm. A staff member has sent thanks to all on Ward 23 for the excellent quality of care given to them during their admission. They described the whole experience as seamless and expressed gratitude for the expertise and support given. A patient emailed to express their grateful thanks for the work of Dr Andrew Ilchyshyn and his team following a procedure.
