2014-2015 Forms Guideline
The purpose of this document is to summarize the key forms, which need to be completed by each and
every student (and their parents) for participation in the FWB High School Band. The forms are
available in paper format in the band room and in downloadable / printable format from the website
(http://www.fwbhs-band.com).
Where do I Turn Forms In?
The ONLY official place to turn forms in is in the clearly marked form receptacles located in the Band
Director’s office. The Band Director’s office has a square column in the middle of the room.
Receptacles will be placed near the column and will be clearly marked for forms collections. On
occasion, we may provide access to the form receptacles at Band Meetings or gatherings.

Do NOT turn any forms into the main office. Our experience has been that many forms turned
into the main office never make it back to our records. This means you will have to fill the form
out again. This includes the volunteer form. We will make a copy of the volunteer form for our
records and submit to the main office.
If you send your forms in with your student, you may want to drop an email for the board to
keep an eye out for the form. Many times, students get busy and the forms do make it to
school but not into the receptacles.

Grading Policy – Students will
receive a grade: 40%
Performances, 40% Rehearsals,
and 20% Assignments. Each
student will start with 100 points in
each category. The rehearsal grade
will be based on attendance during
and after school. Points will be
deducted for unexcused absences.
Excused Absence Criteria

4) Emergency circumstances
which, in the judgment of
the band director constitute
good and sufficient cause
for the absence.
Whenever possible you must give
prior notice of an absence. If prior
notice is not given, it is your
responsibility to bring a note from
your parent or guardian stating the
reason for being absent from a
rehearsal or event. All absences
will be assumed unexcused until
the student takes the proper steps
to have the absence excused.

Drug Free Workplace

FWB High School Parent Volunteer Information Sheet
The FWB High School Band Parents are a key part of the success of the band. The funds in the annual
band budget which the students and parents must raise money for is over $50,000. We need everyone
to help by volunteering time helping with the band and fundraising. Students will raise money for their
accounts, which cover their fees and band trips. Parents will raise money for the General Band
Account, which covers the over $50,000 per year required to keep the band running. The money covers
costs for items ranging from transportation, instruments, repairs, clinicians and much more.

Student activities in-county/off-campus: Parent or guardian shall complete the parent permission portion of the
form for each activity. The form is not required to be notarized for in-county on/off-campus activities.

Student activities requiring off-campus/out-of-county travel: Parent or guardian shall complete the form in its
entirety and have the form notarized for each activity, unless the form has previously been under the multiple off-campus
activities guidelines.
C.
Student Activities requiring multiple off-campus/out-of-county travel: Parent or guardian shall complete the
form in its entirety and have the form notarized. Completion of the form may be used for all related activities (example:
band trips scheduled for the school year).

The Cox Communications Company has generously agreed to sponsor the 38th Annual Live
Television Auction for the Fort Walton Beach Viking Band. This year the auction will be held
from 2 pm until 10 pm on Saturday, December 6, 2014. Not only will the Auction be live on
Cox channel 6, but we will also provide bidding options over the Internet. The Viking Band
would like to thank you for your past support, and once again enlist your participation in this
great event. If you have not participated in the past and would like to be part of this year’s
auction, we welcome you!
In return for an item or service totaling a minimum of $35.00, you will receive prime advertising
in which your items or a facsimile will be shown on television. Information you provide about
your business or product will be promoted by our announcer when the item or service is
presented for auction. If you have an item or items to donate, please contact me at
[email protected] or Stephen Hopkins at [email protected] or 850-6510341 and leave a message. A band parent representative will be in contact with you to pick up
your donation / item, check your advertising information, and provide you with a tax receipt.
The funds that are raised through this auction are used to maintain the quality program that has
earned us the title “The Pride of the Miracle Strip”. We are proud of the efforts that our children
put forth and show our pride by supporting the band in all its activities. The Band Parent
Association uses the funds raised to financially support the band. The uniforms they wear, the
instruments they play, and all the other equipment they must have to put on a performance, are
purchased with the funds the association raises. Our band program currently has over 185
students actively participating, and as exciting as this is, it also takes much fundraising to support
them. The Auction is our largest fundraising effort, and this year we are trying to make it the
best ever.
Your contribution to the auction not only provides you with an excellent means of advertising
your business within the community, it also enables our organization to continue to provide the
financial support to the Fort Walton Beach Viking Band.

Thank you for your support of the Fort Walton Beach Band Parent’s publication of the 2013 Football Program. The
following information will help us, as well as the printer of the program with the proper size and quality of your ad:
Each ad will be contained within a 1 point size line border. Correct sizes of ads are as follows: (all measurements
are in inches)

Advertisements will be accepted in the following formats:
1. Camera Ready Artwork – This means that the ad is in black and white with sharp edges. No grayscale or
gradient fills or pictures. If you wish to submit a picture to go with your ad, please provide the photo or a high
resolution .tif or .jpg file on a disk. (A high resolution means at least 300 dpi). If you wish to provide camera
ready copy with grayscale or halftone pictures, they must be printed with an 85 line screen.
2. Ads submitted on disk – Ads can be submitted in either PC or MAC format. They should be saved in high
resolutions (at least 300 dpi) and in black and white or grayscale for black and white pages. Provide the ad in
either .tif, .jpg or .eps format. If you are submitting your ad in .eps format, make sure it is in a high resolution
.eps with any font converted to curves. If in question, provide the fonts, and picture links or files along with
your finished ad on the disk. We request that a printed copy of your ad be included so the printer can reference
it during layout and design.
3. Full Color Ads – Ads can be submitted in either PC or MAC format. Please provide us with a disk that has
your ad saved in .tif, .jpg or .eps format. Make sure that the file is saved in high resolution (at least 300 spi) and
the colors are in CMYK (not RGB). Provide any fonts, pictures, clip art, etc. on the disk. The important thing
here is that the ad is saved in high resolution and in CMYK colors. We request that a printed copy of your ad
be included so the printer can reference it during layout and design.
4. Custom Designed Ads – If you wish, we can custom design an ad for you. There will be an additional charge
for this depending on what you are looking for. If we are designing the ad for you, we require additional time
for the design work. Please submit your information in a timely manner so proofs can be reviewed and
changes made before the deadline.

The FWBHS Band Trip the 2014-2015 Year is to San Antonio, Texas from April 8 – 12, 2015.
The trip includes:
 Performance at the Alamo Showcase of Music.
o Professional recording of all concert performance. On-stage performance review of each
concert performance.
 Three (3) Nights lodging in a DELUXE San Antonio area hotel.
 Three (3) Complete Breakfasts at the hotel and One (1) Complete Breakfast during Bus Trip and One
(1) Complete Lunch during Bus Trip.
 One (1) Six Flags Meal Voucher and One (1) Schlitterbahn Meal Voucher.
 One (1) complete dinner with $10 play at Dave & Busters.
 One (1) day admission to Six Flags Fiesta Texas.
o You may NOT use Annual or other passes in lieu of the package admission tickets.
o San Antonio theme parks are seasonal.
 One (1) day admission to Schlitterbahn.
 A visit to Historic San Antonio River Walk.
 Two (2) luggage tags for each performance participant.
 Uniformed Security to patrol student occupied floors from 11pm-5pm nightly.
 Services of a Travel Service Host with 24-hour availability. (Optional gratuity NOT included)
 Liability Insurance and all taxes and applicable fees included.
 Date: April 8th through the 12th.
 Chaperones – We are accepting chaperones on the trip. Sign-up for chaperones is September 16th at
the Parent Meeting (7pm). The board will meet and select the final for the trip.
 Guests – We are NOT accepting guests on the trip. NO Guests on this trip.
 Registration and 1st payment due July 15th at the Parent Meeting (7pm).

Auditions for the FWBHS Band
The audition will encompass three categories 1) Scales 2) Sight-Reading 3) Prepared Exercises
Each area will generate a raw score that will be converted to a 100 point scale so that the three areas will be weighted equally.

12 Major Scales should be performed in as many octaves as possible from memory and completed within 2 minutes.
Scales should be performed complete with arpeggios and at a minimum tempo of MM quarter note = 120.
Scales should be performed in complete octaves.
The scale should be performed in the pattern published by the Florida Bandmasters Association.
Scales should be performed chromatically, starting with the concert key of: G, Ab, A, Bb, B, C, Db., D, Bb, E, F, Gb
Chromatic Scale in the ranges listed below. The scale will be performed in even sixteenth notes at a minimum tempo of MM quarter note = 120.
The scale will tongued ascending and slurred descending where applicable. Scale is to be memorized.

Sight-Reading (100 points)
A short sight-reading exercise will be performed to demonstrate the students reading ability. The student will have 30 seconds to study the piece.
Percussionists will sight-read on snare and xylophone
The rubric listed for the prepared exercises will also be used for sight-reading
The 50 point rubric will be converted to a 100 point scale
Prepared Exercises (100 points)
if two are chosen then each exercise will be worth 50 points
if only one exercise is chosen the exercise will be worth 50 points total raw score and then converted to a 100 point scale.

The Fall Audition will be the same as the exercises chosen for the Okaloosa County Honor Band Audition (published in October)

Excused Absence Criteria
1) Personal illness
2) Death of a relative
3) Observance of a religious holiday
4) Emergency circumstances which,
in the judgment of the band director,
constitute good and sufficient cause for absence from band.
Whenever possible, you must notify in writing or call the band office to give
prior notice of absence. If prior notice is not given, it is your responsibility to
bring a note from your parent or guardian stating the reason or excuse for
being absent from a rehearsal or event.

Grading Policy
Students will receive a rehearsal grade (40%), performance grade (40%), and
assignment grade (20%) for each 9 weeks. Rehearsal grade will be based on
attendance to class, night rehearsals, and weekly sectionals. Under the
rehearsal grade, unexcused absence from class will result in minus 5 points,
an unexcused absence from a sectional will result in minus 10 points, and an
unexcused absence from a rehearsal will result in minus 25 points (an
equivalent to 10 points on overall grade). Under performance grade, an
unexcused absence will result from minus 50 points (an equivalent to minus
20 points on overall grade) to possible removal from the program. Assignment
grade will be given based on music pass-offs.
