On Sunday, March 26, 2017 at 1828 hours, I responded to 9800 Meeker Road in Butler Township,
Montgomery County, Ohio, in reference to a hit and run crash that had just occurred at Frederick Pike
and Dog Leg Road.
Upon arrival, I met with Judith Smith, who stated that she was on Frederick Pike and stopped at the
top sign for Dog Leg Road. Mrs. Smith advised that a silver Chevrolet S-10, bearing Ohio registration
GME4570, rear ended her.
Mrs. Smith and her passenger, Larry Smith, exited the vehicle and spoke with the driver, David Terry.
Mrs. Smith and Mr. Terry agreed to pull over to Stillwater Church lot and exchange insurance
information. When Mr. Terry agreed to pull into the lot, both drivers entered their vehicles. Mr.
Terry then proceeded north on Frederick Pike without providing insurance information.
Mrs. Smith provided me Mr. Terry's license plate number. When I contacted Mr. Terry at his residence,
8500 Meeker Road, he advised me that he did rear end Mrs. Smith's vehicle. I asked Mr. Terry why he
did not exchange his insurance information with Mrs. Smith, and he could not give me an answer. I
asked Mr. Terry if he had vehicle insurance, and he stated yes, but he could not provide a copy to me.
I issued Mr. Terry a citation for assured clear distance ahead. I advised Mr. Terry that he had a
mandatory court appearance on Monday, April 3, 2017 at 0830 hours at the Vandalia Municipal Court. I
advised Mr. Terry that it was because he could not provide insurance coverage. Mr. Terry stated that
he understood. I then provided Mr. Terry with a copy of the citation.
There was nothing further to report at this time.
