Please come one half hour earlier to register.
Our building is located on the southwest corner of 8 th Avenue and 24 th Street. If you have
any questions or will not be able to make this appointment, please give us a call at 212-4630101.
Please be aware that patients who arrive one half hour later than their scheduled appointment time
will NOT be seen by the physician and will be rescheduled for another date.
In order to make this process as efficient as possible, we ask that you
1. Complete and bring the enclosed History Questionnaire to this appointment
2. Bring all of your medications
3. Bring your insurance cards
4. If you have not changed your Primary Care Physician (PCP) please contact your
insurance carrier right away to make the physician, named above, your PCP before your
first visit
5. If you are seeing this physician as a specialist and you have an HMO be sure to obtain a
referral to bring to your appointment or fax to our office at 212-463-0952
6. Bring a copy of your Health Care Proxy if you have one
7. Bring a family member to this appointment if possible
