EHR Problem List Update – June 2014 Exhibit 2 Problem List migration to IMO Meaningful Use requires the Transition of Care Summaries (A CCD or Continuity of Care Document in an XML format) to have a Problem List in a SNOMED (Systemized Nomenclature of Medicine) nomeclature. Physicians still use ICD9 for billing purposes. All SNOMED problems will not map to ICD9, thus would disrupt Problem List utilization for conversion to diagnoses. The solution is to leverage IMO (Intelligent Medical Objects) nomenclature, which has a 1:1 conversion between ICD9, ICD10, and SNOMED. When problems are listed in an IMO terminology, they can be converted into ICD9/10 for billing as well as SNOMED for the Problem List that populates the Transition of Care Summaries. What is SNOMED? This is an acronym for Systematized Nomenclature of Medicine – Clinical Terminology. It is a systematically organized, computer processable collection of medical terms providing codes, terms, synonyms and definitions used in clinical documentation and reporting. It is recognized throughout the U.S. and internationally, and it is available at no cost through the National Library of Medicine. Using SNOMED-­‐CT enables providers and electronic medical records to communicate in a common language, thus increasing the quality of patient care across many different provider specialties. What is IMO? Intelligent Medical Objects is a significant improvement over typical ICD9-­‐based controlled vocabulary because it allows clinicians to use the language that is most familiar to them to document patients' encounter diagnoses, problem lists and medical histories. Clinical diagnosis and problem vocabulary which contains synonyms, acronyms and lexical variants that are commonly used by clinicians, coders and patients. ~170,000 terms/concepts representing 16,000+ ICD9-­‐CM codes & terms. Cross-­‐linked with the ICD textual descriptions (as published by CMS), SNOMED CT, MeSH, and UMLS where possible, and is represented in both hierarchical and semantic classifications.
