Patient Information Gynaecology Outpatients Department MYOSURE PROCEDURE WHAT IS A POLYPECTOMY? A polypectomy is a simple Hysteroscopic procedure that removes tissue, such as fibroids and polyps, and requires no incisions. The doctor will place an instrument (hysteroscope) into your vagina and then through your dilated cervix to visualise the inside of your uterus in order to identify a polyp. A tool is then inserted through the hysteroscope to remove the tissue. Once the tissue causing your abnormal bleeding has been removed, your period may go back to normal. This is an ideal treatment option to reduce your heavy or abnormal bleeding caused by polyps, while retaining a fully functional uterus. WHAT IS THE MYOSURE PROCEDURE? The Myosure procedure is a way to remove polyps in the uterus. Myosure is a good treatment to reduce heavy or abnormal bleeding symptoms whilst keeping your uterus. Myosure can also be used for the removal of small fibroids. WHAT ARE UTERINE FIBROIDS? Fibroids are benign growths in the muscle wall of the uterus. Another medical term for fibroids is ‘leiomyoma’ or just ‘myoma’. Fibroids can occur individually, or there can be many of them in the uterus. They can be as small as an apple seed or as large as a grapefruit. There are several different fibroid types, classified by where they grow. The Myosure procedure is used to remove submucosal fibroids and polyps, which are inside the uterine cavity WHAT ARE POLYPS? Uterine polyps are small protrusions of tissue that grow on the uterine lining (endometrium). They are overgrowths of the same kind of cells as the lining itself and may appear as finger-like projections or little mushrooms. As they grow, they become fragile and bleed, and as such, they are a common cause of abnormal uterine bleeding. WHY IS MYOSURE RIGHT FOR YOU? This procedure is carried out using local anaesthetic. The local anaesthetic is given by injection around the neck of your womb/cervix. Once the local anaesthetic is effective, which may take a few minutes, the neck of the womb is gently stretched open. A special instrument called a hysteroscope, which has a small camera on the end, is passed through the neck of the uterus, via the vagina. Fluid is passed into the uterus to distend the uterine cavity to give a clear view. The Myosure device will be inserted through the hysteroscope and the polyp (or small fibroid) will be removed by cutting and then suctioning through the Myosure device passed through the hysteroscope.. HOW SAFE IS THE MYOSURE PROCEDURE? Unlike the removal of fibroids through the abdomen, a hysteroscopic myomectomy requires no cuts or incisions in the skin. The recovery period is quick because there is no need for general anaesthesia, and in a clinical study, the overall complication rate was less than 1% for myomectomies and polypectomies. Complications associated with the removal of fibroids in this way are rare. The potential complications include: Perforation when the cervix is dilated Fluid overload Mild cramping and discomfort Bleeding Infection The overall rate of complications with this type of procedure is less than 2%. HOW EFFECTIVE IS THE MYOSURE PROCEDURE? Overall effectiveness for reducing heavy bleeding caused by submucosal fibroids is greater than 90%. There is a recurrence rate of less than 20% after five years. BEFORE THE PROCEDURE You must ensure that you are not pregnant at the time of the procedure. If you have missed a period and think you may be pregnant then we will not perform the procedure until we have carried out a pregnancy test in the department during your consultation with the Doctor. The procedure can be performed at any time in your menstrual cycle; even during a period, but not if your period is heavy. If you think that you will be in the middle of a heavy period at the time of your appointment please contact us on the number below to rearrange. We suggest that you take two 200mg tablets (400mg total) of Ibuprofen 2 hours before your appointment if you are not asthmatic or allergic to Ibuprofen or aspirin. If you are asthmatic or allergic to this medication, take two 500mg Paracetamol tablets (1000mg total) Myosure Procedure Page 2 of 4 Patient Information WHAT HAPPENS AFTER THE EXAMINATION? It is usual to have some bleeding at first, which will gradually change in colour from red to brown, and may last for about a week. Avoid using tampons until your next period, to reduce the risk of infection. You can use sanitary towels. If the bleeding becomes heavy and smells offensive please contact your GP or telephone the helpline numbers under the section ‘Should I require help or advice’ You may have period-type pain for a few hours after the examination. You can take mild pain relief such as paracetamol if you need it. You may experience more ‘crampy’ period like pains if you have had a coil fitted. Note: if you have been given or advised to buy pain relief. All medication has possible side effects (e.g. dizziness, sickness, indigestion, constipation or itching). Please read the information leaflet enclosed with your pain relief. Do not go swimming, and have showers instead of baths until the bleeding stops. Your next period will be slightly heavier than usual Avoid sexual intercourse/activities for at least two weeks or until the bleeding stops: to reduce the risk of infection. You can return to work and resume normal daily activities when you feel comfortable – usually after a few days. CONTRACEPTION Please continue to use your normal method of contraception before and after your operation unless otherwise instructed by the doctor. If you have been using the coil (IUCD) for contraception or bleeding control, this may be removed during the procedure if you wish so and you may be advised to choose a different method. If you would like to have another coil fitted following the procedure, please discuss this in advance with your doctor. If a coil is fitted please read the information leaflet you are given, and ensure you attend your appointments for a ‘coil check’.
