coverage.
Premiums must be paid in full with
cash, check, or credit card ONLY.

feature and save 5% on your premium.
You are free to cancel the plan at anytime, but there are NO REFUNDS
of any kind.
You are expected to pay your full portion for treatment AT THE TIME
OF SERVICE.
If your account is not in good standing,
we may cancel your plan, or require an
added $50.00 re-enrollment fee.
