Noyes Health is proud to issue the 2016 Annual Community Service Plan to the
New York State (NYS) Department of Health (DOH) and the communities we serve,
demonstrating our commitment to meeting the health care needs and priorities
defined in the most recent Livingston County community health assessment.
2016 was an amazing year that included several milestone accomplishments to
improve the health of communities we serve:
 Noyes Health in Dansville and Jones Memorial Hospital in Wellsville officially
joined UR Medicine January 2016.
 Ann and Carl Myers Cancer Center: construction was completed and the Center
opened January 2017
 Noyes Mental Health and Wellness Clinic opened newly purchased and
renovated location in Dansville December 2016

The Community We Serve
Noyes Health serves a rural community in the New York Finger Lakes region. Our primary market share zip code
report indicates that Livingston County is our health care system’s primary service area and focus of this report.
We also serve residents in the surrounding counties, including: Steuben, Allegany, Wyoming and Ontario respectively.
In 2016, Livingston County was ranked #12 according to the National County Health Rankings and Roadmaps report.
The annual Rankings provide a revealing snapshot of how health is influenced by where we live, learn, work and play.
They provide a starting point for change in communities.

Livingston County covers 640 square miles and has a population of 64,867

The percentage of residents who are 65+ continues to grow annually and 2015 data indicates the rate has
increased from 15.8% to 16.3% of total population

According to the US Census Bureau, 18.4% of Livingston County residents are below 18 years of age, 91% are
non-Hispanic White, 3% are Hispanic, and 2% are Black Non-Hispanic.

6.1% of homes speak a language other than English

Median household income: $51,734 with 13.2% living below poverty level

Noyes Cares About the Community We Serve
Walk to End Alzheimer’s: the Noyes Cares Team walked in October at Geneseo walk.
This mighty team of 9 raised $1,550 to support Alzheimer’s research and support services/
programs in our county!!! Noyes Cares...about families facing Alzheimer's in our community!!
ARC Blood Drives: in collaboration with the Dansville Rotary Club, Noyes Hospital hosted six in 2016 that resulted
in 156 units of blood donated.
Look Good Feel Better: is a free program designed for women dealing with hair loss and skin
changes due to cancer treatments. The American Cancer Society partnered with Noyes, who
hosted four quarterly programs in 2016 and 15 women attended.

Noyes Memorial Hospital
A 67-bed acute care hospital located in Dansville providing medical and surgical inpatient/outpatient services that
include: emergency department, obstetrics/gynecology, radiology, laboratory, rehab services, pain management and
physician services. The newest service at Noyes Hospital is the Ann and Carl Myers Cancer Center that opened
January 2017. A variety of community outreach programs are also provided: prenatal care support, wellness/education/
prevention programs, eldercare support services, and chronic disease self-management programs.
Noyes Health Services
An ambulatory care facility in Geneseo that provides outpatient surgical,
radiology, laboratory, rehab, pain management, and physician services.
Noyes After Hours
Walk-in urgent care clinics are located in Geneseo and Dansville. Both locations serve
patients seven days/week, evenings and weekends, when physician offices are closed.
Noyes Mental Health and Wellness
An out-patient mental health clinic that provides individual, group and
family counseling services. Services now available at seven office/
school locations, including: Dansville (2 sites), Canaseraga, Wayland,
Nunda, Dalton, and Geneseo.
Services at newly purchased and renovated location started December
2016 where they now have 18 counselors and 3 psychiatric providers.
Noyes Center for Kidney Disease and Dialysis Center
A 16-station outpatient chronic dialysis and renal disease
management center in Geneseo.
Certificate of Need to be filed with NYSDOH in 2017 for 6-station
dialysis unit at the Livingston County Center for Nursing and Rehab

Community Health Needs Assessment and Plan
Noyes Health, the Livingston County Department of Health, and the Genesee Valley Health Partnership
(GVHP) were the collaborating leaders who conducted a comprehensive Community Health Assessment
(CHA) in 2016. Population health data was provided by the Finger Lakes Health System Agency for review
and assessment. With the support of the S2AY and GVHP rural health networks, we also engaged
community members through focus groups to solicit input regarding local community health concerns, met
with key health and human service providers to obtain their input, and conducted an online survey among
key stakeholders regarding our current Livingston County public health system.
Noyes Health and the Livingston County Department of Health then completed the 2016-18 Community
Health Improvement Plan/Community Service Plan (CHIP/CSP) demonstrating our partnerships and
commitment to meeting the physical, mental and emotional health needs of the communities we serve.
Now the implementation of the CHIP/CSP is overseen by the Genesee Valley Health Partnership (GVHP).
GVHP is a non-profit, rural health network of more than 40 organizations that are either based in Livingston
County or provide services to Livingston County residents. The GVHP is funded by an Office of Rural
Health grant and is a 501(c)3 organization. Their mission is to improve the health and well-being of the
Livingston County community through collaboration, education and prevention.

At the completion of the Community Health Assessment, the following three NYSDOH Prevention Agenda
health priorities were identified in Livingston County:
1) Chronic Disease Management: with focus on Obesity Prevention
2) Promote Mental Health and Prevent Substance Abuse
3) Safe Communities: with focus on Fall Prevention among the aging population

Prevention Agenda Goal #1: Chronic Disease Management Achievements
Chronic Disease Self-Management Programs
Noyes Health manages three evidence-based Chronic Disease Self-Management education programs in Livingston
County, with Rural Health Grant funding support. Classes are lead by Peer Leaders in locations throughout the county.
Class options include: Living Healthy NY, Living Healthy with Diabetes, and Living Healthy with Chronic Pain.
Noyes Diabetes Education Program
The Noyes Diabetes Education Program is recognized by the American Association of Diabetes Educators. Services are
provided by RN Diabetes Educators and include: one-to-one counseling, FREE monthly classes, insulin pump starts and
management, continuous glucose monitoring screenings, and community education.
Primary Stroke Center: Get With The Guidelines®-Stroke - Silver Achievement Award Winner
In May 2015, Noyes Memorial Hospital became a Primary Stroke Center as designated by The NYS Department of
Health. In 2016, Noyes achieved the Silver Achievement Award from the American Heart Association and American
Stroke Association. Time is vital when assessing for and being treated for strokes
and having a Primary Stroke Center insures the community can get their stroke
care close to home with optimal treatment and outcomes.

Obesity Prevention
(Prevention Agenda Goal #1 cont.)
Day Care Gardens: The Rural Health Grant funded “Kids Dig Dirt” program
has been supporting establishment of gardens in local day care centers since
2010 Noyes Wellness partners with local day care centers to grow gardens
that are taken care of and harvested by the children at the center. The goal is
to increase daily veggie servings and promote healthy eating in pre-school
age children. In 2016, four raised bed gardens were built at Great Expectations
Child Care Center in Lima where 26 children and their
families were impacted. Post survey results indicate the children increase their veggie intake
and are more likely to try new vegetables.
Breastfeeding Initiatives: Noyes Birthing Center continues to expand breastfeeding
support initiatives because it impacts obesity prevention throughout the life span, while also
contributing to numerous other positive health outcomes. Noyes now has five Certified Lactation
Consultants and one International Board Certified Lactation Consultant on staff. Noyes has
been active referral partner with the local Baby Café, a drop-in center at the Dansville Public
Library, with services to support breastfeeding mothers.
Prevention Agenda Goal #2: Mental and Emotional Health Achievements
Noyes Mental Health Services
As community need for mental health services grows, the Noyes commitment to meeting this
growing need resulted in the purchase of a new building in Dansville for a new Noyes Mental
Health and Wellness Clinic, that opened 12/2016. Mental health services are also provided at
seven other locations throughout our service area, including schools and physician offices.
Staffing: three fulltime Psychiatric Providers (one MD who is a Child & Adolescent
Psychiatrist and two nurse practitioners), 18 full time therapists and 3.2 clerical staff.
Crisis Management: Daily Walk-In Crisis services at main Dansville Clinic.
Outcomes: Patient visits increased from 13,000 visits to 15,500 visits/year in 2016 (19% increase)
Social and Emotional Health Committee
Their goal is to strengthen social and emotional health. The committee works on prescription misuse/substance abuse
and suicide prevention initiatives. Noyes Mental Health supported the following task force initiatives in 2016:
Suicide Prevention Task Force: Suicide rates in Livingston County continue to be highest
in our nine county region. This task force continued to work on suicide prevention education
initiatives, including: SAFE Talk Suicide Prevention Training, Toolkits, annual Candle Light Vigil
for families of suicide, and movie theater, bill board, and print ads on suicide prevention.
Prescription Drug Disposal Task Force: Proper prescription drug disposal in the county
was addressed. Two new Med Return boxes were placed in Livingston County in 2016, for a
total of three current locations: Caledonia, Dansville, and Geneseo.
Alcoholics Anonymous (AA) Meetings
Noyes Hospital provides conference room space for weekly AA and Narcotics Anonymous meetings, at no charge to the
group. These efforts support community need to help those living in our service area who struggle with addiction issues.
Prevention Agenda Goal #3: Fall Prevention for Older Adults
In our 2016 Community Health assessment, seniors ranked falls as one of their primary health care concerns. Noyes is
an active partner on the Livingston County Fall Prevention Committee, working with the Office for the Aging and
Department of Health staff. New and expanded education and outreach strategies for 2016 included:
 Annual Fall Prevention Workshop, Education Rack Cards, and Outreach Ads
 Matter of Balance: fall prevention education classes
 Tai Chi: strength and balance classes
 Chair Yoga and Thera-band exercise demonstrations at Senior Nutrition Sites
 Introduction of Home Safe Home: a fall risk home assessment/education program for seniors

2016 Noyes Community Outreach Program Outcomes
Noyes is committed to improving the health of the communities we serve and our Community Outreach Team provides oversight and
programs. This Team is made up of skilled nursing professionals, health educators, program coordinators, and support staff, all
working together to enhance the overall health, knowledge, and safety of people living in our service area. Our programs have
evolved and expanded to meet community needs based on the most recent Community Health Assessment completed in 2016 that
identified the top three NYSDOH Prevention Agenda priorities. (listed on pg. 4)
Noyes First Steps: Provides Medicaid prenatal health support services that include: facilitated insurance
enrollment, prenatal education, nutrition counseling, childbirth classes, referrals, and Case Manager
Services to address needs of high-risk pregnant women that could impact healthy birth outcomes.
113 Total Clients Served with 93% Healthy Birth Weight Rate
Community Wellness Screenings: Provides free blood pressure screening clinics with education and physician referrals,
cholesterol screenings, weekly Wellness articles in local papers, Wellness Education speakers/programs, as well as management of
the Noyes Worksite Wellness program for 550+ employees. Annual encounters: 2826 (6% increase)
Community Education: Classes included: Medical Terminology, Smoking Cessation Education,
Phlebotomy Training Program, Child Birth Education Classes, and Community Speakers Bureau.
Participants/Students attended: 252 (63% increase)
Diabetes Education Program: Our American Association of Diabetes Educators (AADE) recognized program
offers individual counseling with Certified Diabetes Educator, free Life with Diabetes classes, and other services, including: insulin
pump starts/support, CGM screenings and glucometers with education. Diabetes enrollments/referrals: 378 (18% increase)
EARS Lifeline: Local Philips Lifeline Program provides a variety of medical alert services, including:
Basic, Wireless, Auto Alert Fall Detector, and the mobile GoSafe. All options support independent, living
for older adults with chronic health issues or disabilities, when they are home and on the go. Lifeline staff
are also members of Livingston County Fall Prevention Coalition that coordinates the Livingston County fall
prevention programs. 887 Lifeline Subscribers served with 758 emergency assists (30% increase)
Philips Medication Dispensing Service: Provides automated medication dispensing service to ensure
the “right medication is taken at the right time” - with Caregiver alerts if medication is missed. Subscribers served: 19 (58% increase)
Livingston County Caregiver Resource Center (CRC): Provides caregiver support
services that include: caregiver counseling, information & referral, respite care services,
education/classes, caregiver support groups, and Annual Caregiver Celebration. Program
partner and funder: Livingston County Office for the Aging. 73 Caregivers/Families served
by CRC and 70+ Caregivers attended the 7th Annual Caregiver Retreat in November.
Chronic Disease Self-Management Programs: Noyes Health successfully coordinated all chronic disease self-management
programs in Livingston County. In this 6-class series, participants get needed support; discover better nutrition & exercise choices;
and learn how to deal with pain & fatigue, get better sleep, talk with their doctor & family about health issues, set goals & make a
step-by-step plan to improve their health & life! Chronic Pain Self-Management classes were newest
class introduced in 2016.
Living Healthy NY: 2 classes, 51 participants registered and 27 completed
Living Healthy with Chronic Pain: first class, 10 registered and 9 completed

Community Service Update from Amy Pollard President, CEO
Meeting our community’s health care
needs cannot be done in isolation. Noyes
Health values the partnerships with
Livingston County Health Department and
the Genesee Valley Health Partnership which represents many health and
human service agencies. Together we assess the health status of our region
and then plan how to address defined needs for care.
Noyes focuses on the priorities of Chronic Disease Management and Mental
Health Promotion/Substance Abuse Prevention. Our diabetes educators,
community outreach service and wellness coordinator are examples of our
efforts to address chronic disease management. By the end of 2016, the new
location for Mental Health and Wellness services was ready to be occupied. The
renovated building on Robert Hart Drive provides updated, private office space
for 22 mental health providers who see up to 1400 visits per month.
Throughout 2016 we watched as the Ann and Carl Myers Cancer Center was
completed. Comprehensive cancer services are now available in our region thus
eliminating the need to travel to Rochester for care…..much easier for patients
and families.
2016 was our first full year as an affiliate of the University of Rochester and was
spent learning the benefits of affiliation which included expansion of clinical
services.
The credit for our growth and success goes first and
foremost to our hospital and medical staff. Our visionary
Hospital and foundation boards along with UR Medicine’s
depth of knowledge ensure our success. And, most
importantly, thank you to our community and donors who
continue to support UR Medicine/Noyes Health.
