Ben Coggins called the meeting to order and opened the meeting with the
CFAC Mission statement and Purpose
Ben welcomed guest. They introduced themselves.
Ginny Hall made a motion to approve the minutes for the month of
February, Cordell Thomas 2nd the motion.

PARTNERS: Whole Person Integrated Care Model
Jennifer Greene- Integrated Care Project Manager
• New initiative from Partners to meet the needs of the whole person.
(Make sure that the medical needs are being met as well as psychiatric
needs, etc.)
• Quadruple Aim: Community as whole can be able see and receive
better healthcare.
• The peer role is going to be one of the strongest components in the
model.
• Everyone involved in the person’s treatment has access to the
information.
• Collaborative Care Model- Holistic Care to facilitate care to the whole
person.
• HUBS are designed to reflect the Community Needs.
• Specific needs that plague your community. This is what is going on in
my community?
• Partners Health Summit will be held on March 31, 2017 at the Hickory
Civic Center from 8:30 am until 4:30 pm.

New DHHS Secretary presented during the session. Had a lot to do with
Health Care Act.
Jaycen Boglar presented.
The Governments budget is out. IDD population expanding to a 1,000
Innovations Waiver slots.
Currently there 11,800 individuals on the North Carolina Waiting List.
Talked about transitioning to Community Living & Supports.
Veterans getting the therapy they need by farming. (Veterans to
Farmers)
Conference call on 03/15/2017 at 7:00 p.m.
The Tide Conference is upcoming on April 23rd thru April 26th in
Wilmington N.C.
Disability Conference will be coming up April 20th, 2017.
Cross function team report Ginny Hall stated that there were not
enough surveys completed in the Needs /Gaps Analysis.
Ginny Hall-Gaston County Collaborative (Family Involvement)
• Community Involvement anything to do with children and their
behavioral health.
• Family Involvement is very important to determine what the needs are
within the community.
• The collaborative meets at the Ollie Harris Center in Cleveland County
the first Wednesday of the month at 12:30 p.m.
CFAC Event

• Executive Committee met to bring back structure to the CFAC
committee also to get recommendations of changes decided upon.
• CFAC Members will be hosting and welcoming our guest.
• Brochure with agenda inside along with biography of the speaker.
• Lunch will be served.
• Vote for the Topic of the retreat.
• Retreat was held on March 18th, 2017 from 9am-2:00p.m. at Tate Blvd.

Health & Human Services.

Her presententation was centered on Team Work. Individuals are
diverse but our diversity makes us stronger. (Better able to build on
other’s experiences).
Forming, Storming, Norming, and Performing.
The common goal of a team is to work towards a consensus but that
may not mean 100% agreement.
You want to know what that common goal is and then break it down
into chunks.
The Core reason for the existence of the team is vital. The Mission/
Vision/ By-Laws; also providing recommendations to the department.
Everyone on the team, in the room has an equal voice.
