Dr. Morris opened the meeting with all Board members present. Howard Flippin,
Executive Director, Ann Booker, Board secretary, and Bill Trice Board Attorney were
also present. Vicki Farmer, Linda Sanders, Varci Middleton-Yates, and Lisa Wooten
were in the audience. Awaiting hearings just outside the Board room were Sal Caputi,
Moustafa Hasan, Adel Saleh, Shahjahan Manhood, David Park, and Haydar Abulut.
Dr. Morris opened the meeting welcomed the guests.
Dr. Hubbard made a motion to accept the minutes as written, Dr. Coffee made a
second. Dr. Smalling made amendment to the minutes. The motion passed.
Mr. Trice informed the Board Dr. Ridings had not reviewed his lease. Dr Ridings will be
notified to come to the next meeting. Mr. Trice informed the Board Dr. Freshour is now
under treatment. Mr. Trice informed the Board that the department of corrections utilizes
Correction Services Inc. to find doctors to work for the Department and place them only.
They do not provide optometric services.
Mr. Shirley gave a financial report on the Board office.
Dr. Hass made a motion to investigate two stores in El Dorado concerning illegal
sales of contact lenses. Dr. Coffee made a second and the motion passed.
Dr. Morris opened and closed the meeting for six hearings concerning illegal sales of
contact lenses. At the conclusion of each hearing the following motions were made.
Dr. Smalling made a motion that a violation had occurred at Sara Jewelers of Pine
Bluff. Dr. Haas made a second and the motion passed.
Dr. Haas made a motion to fine Shahjahan Manhood d/b/a Sara Jewelers
be fined five thousand dollars to be paid in 30 days. Dr. Coffee made a second
and the motion passed.
Dr. Smalling made a motion that a violation had occurred at Beauty Gallery of
Forest City. Mr. Spears made a second and the motion passed.
Dr. Haas made a motion to fine Sal Caputi d/b/a Beauty Gallery of Forest City be
fined five thousand dollars to be paid in 30 days. Mr. Shirley made a second and
the motion passed.
Mr. Spears made a motion that a violation had occurred at Sunny Shade of
North Little Rock. Dr. Smalling made a second and the motion passed.
Dr. Haas made a motion to fine Haydar Abulut d/b/a Sunny Shade be fined five
thousand dollars to be paid in 30 days. Dr. Smalling made a second and the
motion passed.
Dr.Coffee made a motion that a violation had occurred at Super Tops &
Bottoms of Little Rock. Dr. Haas made a second and the motion passed.
Dr. Haas made a motion to fine David Park and Eunj Park d/b/a Super Tops &
Bottoms of Little Rock be fined five thousand dollars to be paid in 30 days. Mr.
Shirley made a second and the motion passed.
Dr. Coffee made a motion that a violation had occurred at Sam's Phones &
Accessories of Forest City. Mr. Shirley made a second and the motion passed.
Dr. Haas made a motion to fine Adel Saleh d/b/a Sam's Phones of Forest City be
fined five thousand dollars to be paid in 30 days. Mr. Spears made a second and
the motion passed.
Dr. coffee made a motion that a violation had occurred at Downtown Express
Mart of Jonesboro. Dr. Haas made a second and the motion passed.
Mr. Spear made a motion to Moustafa Hasan d/b/a Downtown Express Mart of
Jonesboro be fined five thousand dollars to be paid in 30 days. Dr. Haas made a
second and the motion passed.
Dr. Flippin presented a list of newly licensed doctors. He presented a formal complaint
filed by a patient to the Board. No action was taken.
Mr. Trice discussed practices left behind after the death of a doctor. He reminded the
Board that only doctors of optometry can hold patient records and practice optometry.
Patients have control of their records. Rental of equipment and or space can be controlled
by an estate.
Dr. Coffee made a motion the Board be paid one days expenses and travel for the
Board meeting. Dr. Smalling made a second and the motion passes.
The next Board meeting will be November 3, 2011.
Dr. Morris adjourned the meeting.
