your MEDICAL CERTIFICATE. If you cannot come for the interview, please contact the TVET
Assistant. Otherwise, your slot will be forfeited.
INTERVIEW PROCEDURES:
1. Proceed to the guard on duty at Gate 1 (TVET Center); Show your Medical Results and get a
number. NO MEDICAL RESULTS, NO ENTRY policy will be applied.
2. Submit your Medical Results to the Nurse at the TVET Clinic;

3. Proceed to the TVET lobby of Braga Building (new building) and look for Mrs. Joanna Li.
Ask for one (1) copy of the Interview Sheet, two (2) copies of the Agreement Form and the
Trainee's Information Sheet (to be filled up at home). Fill-up the Interview Sheet and
Agreement Forms.
4. Proceed to the External Relations Office for the 1st interview.
(look for Mr. Armando Z. Marquez)
5. Proceed to the Office the Spiritual Moderator for the 2nd interview.
(look for Fr. Ben Hur Borja, SDB)
6. Proceed to the Office of the Technical Director for the final interview
(look for the TVET Assistant)
7. You may ENROLL after completing the interview.
Bring the following:
 Interview sheet
 Assessment forms
 Trainee's Information sheet (properly filled-up)
 Php2,000.00.
