need everyone to do to make sure
we can claim Gift Aid on eligible
donations. Follow these notes to
make sure your sponsors complete
all parts of the form correctly.

Write your name in full. Nicknames or incomplete names
mean we can’t collect the Gift Aid on donations.

Complete your full home address, not your work one,
including flat/house number, road, town and postcode.
We won’t be able to claim the Gift Aid on your donation
if one of these is missing.

Write down how much you are donating, so that we know
how much Gift Aid to claim.

Fill in the date you give your donation to ensure that the
Gift Aid is valid.
