The tele-interview is an interview conducted over the telephone by a specialist nurse who will gather information about your health and medical history. All interviewers are qualified nurses so you can be assured that the interview will be conducted in a confidential and professional manner. The nurse will also ask questions about lifestyle, occupation, pastimes and your immediate family medical history. The interview will take about 30 to 45 minutes. Please note that the calls will be recorded. Why Am I being Interviewed? The interview will help speed up your application and ensure we have a clear understanding of the information you have provided. The information you provide will be treated in the strictest confidence and will only be used in assessment of your application and any future claims for benefits. How Will You Be Contacted? Medicals Direct (a specialist company working on behalf of Holloway Friendly) will initially contact you to arrange a convenient time for the interview to be carried out. If you have not been contacted within 3 days of application or have been away or out of touch you should contact us on the free phone number 0800 716654 Alternatively you can schedule your own appointment at www.wearemdg.com/book-an-appointment Please have your Tracking Reference Number available when you enter this site. You can reschedule an appointment from this site if greater than 24 hrs notice is provided, otherwise contact Medicals Direct on 0845 056 0538 Once scheduled a nurse from Medicals Direct will contact you to conduct the interview at the specified time and date. Medicals Direct nurses can undertake interviews from: Ÿ 9am to 8pm Monday to Thursday Ÿ 9am to 6pm Friday Ÿ 9am to 3pm Saturday If you have call barring on your telephone, please arrange for this to be removed to allow Medicals Direct to call you. If the call barring cannot be removed, please call Medicals Direct on 0845 056 0538. You should be able to speak freely and have sufficient time to complete the interview. It may therefore be easier to use a landline rather than a mobile. Please note the interview cannot take place if you are driving a motor vehicle or operating any type of machinery. What Do You Need To Do To Prepare For The Interview You can help to ensure the interview is as short as possible by having the following information available when Medicals Direct call: Ÿ Details of any medication you are taking (including name and dosage). Details of any past and/or present medical conditions suffered from and/or diagnosed (other than very minor ailments such as the common cold). Ÿ Any tests or investigations e.g. blood pressure or cholesterol tests. It would be helpful if you phone your GP or whoever carried out the tests to get the results and date taken. Ÿ Details of any serious medical conditions, such as cancer, heart attack or stroke suffered by a member of your immediate family (mother, father, brothers, sisters, half brothers and sisters) and their age when diagnosed. Ÿ Your height, weight and waist measurements and collar size or dress size. If you are not sure of these measurements, please check them prior to the interview. Ÿ The name, address and telephone number of your GP. If you have been registered with your current GP for less than 3 years please also provide your previous GP’s name, address and telephone number. If you are not sure whether something is important, you should mention it, the interviewer will assist you with any questions you may have. What happens After The Interview? You will be sent a copy of the tele-interview for you to check. Should any details be incorrect or you need to add information to the interview, please correct the report and sign and date the alteration before returning it to us within 10 days as any changes may affect the terms we can offer you. If you have made changes and returned them but have not heard from us, please contact our Underwriting Department on 01452 782760. If there are no changes to the tele-interview report, then we will write to you with our decision. In some instances we may need to request further medical information from your doctor or require you to undergo a medical examination. This may take some time to arrange and we will let you know if we need to do this. Why Is It Important That I Provide The Right Information? The interview forms part of your contract of insurance and is recorded. You must answer all questions accurately and completely and to the best of your knowledge and belief. If you do not it could mean that we do not pay your claim. If you are in doubt as to whether any information should be disclosed to us, then you should disclose it.
