Chorley Leisure Centre Brookhaven is located just off Gough Lane, approximately 2 miles from Bamber Bridge and within 1.3 acres of landscaped gardens. There are excellent rail and bus networks and limited free visitors’ parking on-site. There are a range of shops, a sports centre and a supermarket in nearby Clayton Brook, all within 20 minutes’ walk of Brookhaven. Cuerden Valley Country Park, less than 2 miles away, has meadows, woodland, lakes and parkland. Chorley, a large market town, is 5 miles from Brookhaven and has a wide range of facilities including a hospital, college, museums, shops, parks and recreational spaces. Your team will support you to access local amenities that are of interest to you. Cuerden Valley Country Park Chorley Town Centre 4 Your Pathway: a better future Brookhaven Rehabilitation and Recovery Service Welcome Guide 5 What you can expect in the first few days We understand that coming into a new service could be difficult and we will offer you the support and assistance that you may need. When you arrive you will be greeted by one of our staff who will show you to your room where you can safely store your personal belongings. When you are ready, a member of staff will show you around and introduce you to everyone. Facilities Your space Bedroom Each bedroom is furnished with a bed, wardrobe and bedside cabinet and has en-suite facilities and access to a small personal safe. We encourage you to personalise your bedroom and you can bring your own items of furniture and belongings so long as they are safe and there is space for them. We kindly request that you keep your room clean and tidy and inform staff if there are any faults/defects in your bedroom. We ask that you do not enter each other’s rooms as these are private areas. If you wish to have a television and/or radio in your room we would appreciate it if you could respect others by keeping volumes to a reasonable level. If having a radio/television results in you having prolonged periods of time alone in your room, staff may discuss this with you. Any electrical items that you bring must be tested by our electrician to make sure they are safe to use. We will need to take a photocopy of the warranty for any new items you buy. Shared space In Brookhaven we have shared areas which include: • • • • • 6 Your Pathway: a better future Lounge Dining room and kitchen Skills kitchen ‘My space’ is a multipurpose room which includes an education area Activity room Brookhaven Rehabilitation and Recovery Service Welcome Guide 7 Facilities Laundry Smoking Smoking is not permitted in any area of the building and it is against the law to smoke indoors. You can smoke only in designated areas outside the building. We provide laundry equipment for you to use which is located in the Laundry Room. This includes a washing machine, tumble drier, iron and ironing board. Bed linen is supplied for you to change on at least a weekly basis. As part of your life skills you are expected to wash your own clothes. If you need support with any aspect of your laundry, please speak with a support worker or member of our nursing team. Telephone We have a pay phone in Brookhaven for your use at any time of the day. You can also retain your personal mobile phone during your stay with us. Internet Brookhaven has internet access in designated areas. If you would like to have access to the internet you will need to agree this with your multidisciplinary team. Food Brookhaven has a kitchen where a choice of fresh meals are prepared daily by our catering team. All menus take into account dietary needs and respect cultural and religious requirements. Service users are involved in choosing menus. Meals are served at the following times: Breakfast: 8.00 am – 9.00 am Lunch: 12.00 pm – 1.00 pm Dinner: 5.00 pm – 6.00 pm Supper: 9.00 pm – 10.00 pm If you would like food outside of these times we have a range of cold foods such as sandwiches, salads, cereals and fruit. Our chef will also prepare a hot meal outside of these times by prior arrangement, although we encourage you to cook/prepare your own meals in the skills kitchen. If you would like support with this, you can talk to a member of our occupational therapy team. Hot and cold drinks are available 24 hours per day. 8 Your Pathway: a better future Visitors We understand the important role that family and friends can have in helping you on your journey to recovery. We will try to ensure visits can be facilitated as flexibly as possible. However, we ask that wherever possible visits are pre-arranged through the Nurse in Charge to ensure there is a suitable meeting room available. Children under the age of 18 may only visit if it is in the best interests of the child. You will need to discuss proposed visits by children with the multidisciplinary team. Leave You will be encouraged to go on escorted or unescorted home visits provided that the risks have been properly assessed. Brookhaven Rehabilitation and Recovery Service Welcome Guide 9 Facilities Advocacy You can have access to a free, confidential and independent advocacy service to discuss any issues that might be of concern to you. The advocate can provide support and help to get your views and choices heard and advise you on any aspect of your care that you are either unsure or unhappy about. Our staff will provide you with details of Advocacy Services. What happens in Brookhaven? Assessment period When you are referred to Brookhaven, you will be assessed by members of the multidisciplinary team to ensure we can meet your needs. Following admission, you will receive more indepth assessment during the first 4 weeks which also enables you to make your own judgement about the suitability of Brookhaven to meet your needs. Recovery STAR We are here to help you in your recovery from mental illness. Recovery usually means changing things in a number of areas of your life so that things work better for you. Making changes isn’t easy but understanding how change works can help. Many people who are recovering from mental illness have found it useful to think about recovery as a journey with different stages. They find it helps to think about which stage they are in and to get a picture of where they are on their journey. We use the Recovery STAR to help you with this. Fire In the event of the fire alarm being sounded, you are required to follow the instructions of staff or make your way directly to the identified assembly area. All fire exits are clearly signposted around the unit and fire extinguishers are located throughout the building. Staff will show you your personal evacuation plan. 10 Your Pathway: a better future Multidisciplinary Team (MDT) A team of professionals including occupational therapists, nursing staff and support workers will support your recovery. You will be registered with a local GP. Brookhaven Rehabilitation and Recovery Service Welcome Guide 11 What happens in Brookhaven? Care Programme Approach (CPA) Active Support Plans Active Pathways is committed to involving people who use our services in planning their care. The Care Programme Approach is the framework for providing care to people over 16 who have mental health problems. It is the way of assessing your needs and planning with you the best way to ensure that your needs are met. You will have your own active support plan which will be developed in collaboration with support staff. This will offer a meaningful way of tracking your progress against each of your personal needs and will be a means of celebrating the achievements you have made on your recovery journey. What does it involve? • • • • An assessment of your health and social care needs You will be offered a written Care Plan detailing the help and support that you will receive Regular review meetings to discuss how your care plan is working and agree together changes that may be needed A named Care Co-ordinator who will work specifically with you We will meet with you to discuss and assess your health and social care needs and the care and support you need. We will support you through group work and individual sessions which will be delivered by staff to enhance your social and emotional development. Supported learning programmes will be delivered within Brookhaven and in community based settings. With your permission, other people who know you well, e.g. your carer, or close relative, may be asked for their views about your needs. Additionally your GP and other professionals involved in your care will be consulted. We will take account of your gender and any issues of culture and ethnicity that are important to you. With your full involvement, we will agree a plan of your care. You will be given a written copy of your Care Plan to sign if you are in full agreement. 12 Your Pathway: a better future Physical healthcare We recognise the importance of good physical health. A named nurse will support you with your physical health needs, in collaboration with our service physical healthcare lead, your GP and other healthcare professionals as required. Risk assessment and management You will be involved in writing your care plan and assessing and managing your own risks. We will help you to understand ways of reducing risk as part of your recovery. Personal medication All medication you bring with you must be handed to the nursing staff who will store it in the treatment room when you arrive or given to family members/carers to take home. You may also look after your own medication in a locked cabinet in your bedroom and administer it yourself. If you would like to participate in our self-medication programme then you should discuss this with your doctor at your next multidisciplinary meeting. Cleanliness and personal hygiene During your stay, we would like to encourage you to maintain a good standard of personal hygiene. Please keep your bedroom clean and tidy. An untidy room can create health and safety, fire and infection control hazards. Please ensure you are appropriately dressed when you are in the main communal areas. Activities and therapies Practical support and guidance The programme helps you to build on existing skills and develop new ones in preparation for living in the community. We will help you to develop practical skills including, using the bus, cooking and self-care. Management of risk Employment and education The therapeutic programme will help you to manage your own risk more effectively. Brookhaven staff will support you to recognise your own risk factors and take appropriate action to ensure a positive response to ensure you can live with less risk to yourself, including vulnerability and self-neglect. The ability to live independently and remain healthy in the community will be largely dependent on your ability to gain meaningful paid employment. We provide a range of opportunities to acquire educational qualifications, enhance a CV, gain interview experience, and participate in voluntary and paid employment. Physical health Preparation for community transition We are here to provide a psycho-educational approach to your physical health which includes; diet and exercise, medication management, giving up smoking, management of physical health conditions, dental hygiene, sleep, impact of stress on the body and lifestyle choices Discharge is the ultimate aim of the service and therefore every effort is made to prepare you for life in the community, including, management of finances, budgeting and paying bills, developing social and family networks, citizenship activities and relapse prevention. Modular Transitional Rehabilitation Programme (MTRP) Dual diagnosis Brookhaven is able to provide a structured approach to substance misuse, including supportive programmes for drug and alcohol issues. We have staff who are trained in the delivery of Cognitive Behavioural Therapy (CBT) approaches and a core team of staff trained in the management of substance misuse. We are using evidence based approaches & models supported by the National Treatment Agency for Substance Misuse. Psychological The service will continue to provide psychological support including programmes for stress, anger and anxiety management. We are able to provide 1:1 Psychology assessment and support on a needs led basis. Reduction in symptoms It is recognised that you may continue to have complex mental health difficulties which will impact on your ability to function in your day-to-day life. This may include a lack of motivation and other negative symptoms. The staff team will work with you and help to support a reduction in symptoms and improvements in your overall quality of life. Medication compliance The ability to self-manage medication within the community is an important part of relapse prevention. Using a psycho-educational approach we will support you to fully understand your medication, the need for compliance and through a structured approach will introduce a self-managed medication programme. 14 Your Pathway: a better future We facilitate a programme known as the MTRP, this is a more advanced programme for those people who do become progressively engaged in their recovery. The programme provides a safe environment where elements of recovery can be explored and developed within a therapeutic group environment. This is a 6 module programme which includes a wide content of material, including: • • • • • • Development of Self Symptom Management Understanding Recovery Employment and Education Family and Social Support Networks Relapse Prevention. The MTRP is facilitator led and workbook based within a group work setting. The group dynamic is a large part of the therapeutic, peer support based, element of this programme as well as other positive aspects such as setting of recovery goals, emphasis on development of social connections, and personal strategies to manage symptoms and foster positive mental health. People who participate in the programme will be expected to enter into a therapeutic contract and commit to attendance and completion of homework exercises. The group is fun but has a serious therapeutic message and forms a large part of the treatment strategy. People who participate in the programme will be assessed before, during and after the programme to evidence progress and the development of new skills. Brookhaven Rehabilitation and Recovery Service Welcome Guide 15 What to bring with you? ✔ Ethnicity & cultural awareness You should bring what you need for day-to-day living. You will have room in your bedroom to store personal belongings but please be aware that space may be limited. We will provide safe and secure facilities for you to store your valuables and money and there is a safe available. However we advise you not to bring valuables or more money than necessary for your day-to-day living. What not to bring Restricted items ✘ At Active Pathways we work on the basis of the least restrictive access to personal property. However there may be restrictions on certain items. For your reassurance your property will be checked and recorded with you. The discrimination, harassment or abuse of another service user, member of staff or visitor on the basis of ethnic origin, race, colour, age, gender, sexual orientation, religious orientation, or any other reason will not be tolerated. Alcohol and illicit drugs Such behaviour causes offence and will be monitored and documented. Alcohol and illicit drugs are not allowed within the service. You may consume alcohol away from Brookhaven. It is a criminal offence to be in possession of illicit drugs. We may take random urine and breath samples to screen for alcohol and illicit drugs and we would expect you to co-operate with these checks for the safety of yourself and other service users. Anyone found in possession of illicit drugs will be reported to the police. Should the discriminatory behaviour continue then the offer of a placement at Brookhaven may be withdrawn and the matter referred to the police and safeguarding authorities for investigation and potential criminal prosecution. Explicit adult content Pornographic magazines and/or books will be individually risk assessed for suitability at Brookhaven. If deemed acceptable, they must be kept discreetly in the individual’s bedroom at all times. The display, exchange, showing to others, watching on television or taping of pornography is prohibited. 16 Your Pathway: a better future Brookhaven Rehabilitation and Recovery Service Welcome Guide 17 Your rights As a person living at Brookhaven you have the right to: 18 • • • • • be treated with dignity • • have your privacy respected • have your social, emotional, physical, psychological and spiritual wishes and needs acknowledged and fulfilled wherever possible • be informed and consulted about any matters that may affect your life in the home • participate fully in the planning of care, and to have any proposed care, treatment or options explained to you, so that you may make decisions about the care that you receive • choose to be alone, undisturbed, and free from intrusion, as far as possible, within the constraints of living with others in a nursing or residential care home; and the right to have all your personal information kept confidential the maximum level of independence, choice and control possible be listened to and supported to express your needs and wants be assisted to maintain confidence and a positive self esteem be shown respect and courtesy and to be treated as a unique and valued individual live in a safe, warm, clean and comfortable environment, where the homes’ philosophy and care practices support you, and enable you to enjoy the quality of your life • maintain links with family and friends; to be supported in doing this, and to have the opportunity to welcome your visitors or, to ask for those people who you do not wish to see, to be excluded • maintain responsibility for your personal affairs and to have confidentiality maintained at all times • have a person independent of Brookhaven, or the Company, represent you over any issue, for example, to raise concerns or make complaints on your behalf • comment and make known any concerns or complaints that you have about the home, or the Company, without feeling under stress or threat Your Pathway: a better future • have access to your care notes, except when your General Practitioner or Medical Consultant has specifically identified that this may be detrimental to your well being • choose a General Practitioner to provide general medical services for you, should they agree to register you on their GP list • have a Named Nurse or Keyworker, who will be accountable for ensuring that you receive the care and support that you need • where practicable, to have your personal possessions and clothing around you, and to have them cared for appropriately • fully exercise citizenship rights, such as your right to vote Compliments/concerns/complaints We welcome feedback from you whether it is positive or negative as a way to continually monitor the quality of the service we provide. There are a number of ways you can feedback: • • • • COMMENTS BOX SERVICE USER SATISFACTION SURVEYS SERVICE USER FORUMS SERVICE USER REPRESENTATIVE Complaints procedure As an organisation, we promote a culture of openness, honesty and transparency and treat all concerns and complaints seriously. When you arrive we will provide you with details of how to make a complaint if you are unhappy or have concerns about any aspect of your care and treatment. If you wish to see a copy of our Complaints Policy, you can request this from the manager. If you, a member of your family or any other person wishes to raise a complaint either orally or in writing you should make your complaint to the Registered Manager. An advocate, member of staff not involved in the complaint, or other person of your choosing can support you to make your complaint. If you are not satisfied with the outcome of a written complaint you can write to the Head of Operations. Alternatively complaints can be made to The Care Quality Commission, or your care manager. We have a zero tolerance of all forms of abuse.
