The Volunteer Home Visitors service is a social support service that aims to assist people to remain living in their own homes for as long as possible. Position Summary This position is responsible for coordinating the Volunteer Home Visitors service and Springwood Neighborhood Centre social support groups, to meet the needs of the Community Home Support Program (CHSP() target group living in the Blue Mountains ( from Valley Heights to Linden). The position involves: Client assessment, care plan and referral; Recruiting, training and supporting volunteers; Matching volunteers with clients; Maintaining contact with other referral agencies; Promotion of the VHV service; Monitoring the Volunteer Home Visitors budget; Recording &reporting service activity levels Overseeing and coordination of all VHV social support sessional, casual & contract workers Promotion and coordination of SNCC social support groups for the target population: fulfilling accountability requirements with Australian Government and those of Springwood Neighbourhood Centre Cooperative Ltd. Networking with the community service sector; Liaison with other relevant community services. Decision making Authority You are responsible for fulfilling your duties within the framework of legislative requirements and Springwood Neighbourhood Centre Cooperative’s policies and procedures. Issues are usually resolved without reference to your immediate supervisor but matters that arise which are outside the policy framework or matters which may potentially escalate to the detriment of the Springwood Neighbourhood Centre cooperative should be reported to the Executive Officer Context The Springwood Neighbourhood Centre Cooperative Ltd provides and maintains a diverse range of community services and programs in response to identified needs within the community of service. SNCC provides services and support for residents and community groups in the Valley Heights to Linden community, as well as a number of Local Government Areas. .. SVHV services the target community from Linden to Valley. SNCC operates in an openly accessible and accountable way, provides service to and advocates on behalf of vulnerable community members, and promotes the development of social inclusion and individual capacity building initiatives and support networks in order to enhance individual opportunities and strengthen overall community resilience. The position is based at the Springwood Neighbourhood Centre Cooperative Ltd and provides coverage across the Blue Mountains hamlets of Valley Heights to Linden. The service is funded by the Commonwealth Department of Social Services; Community Home Support Program & is subject to ongoing funding. Additional information General Conditions This position is covered under the Social, Community, Home Care and Disability Services (SCHADS) Award 2010. Service coordination and support Recruit, orientate, induct, train and support Volunteer Home Visitors to enable them to enact their roles within the community environment Match home visitors with clients of the service Plan & conduct appropriate training programmes for volunteers to ensure quality service provision is sustained Convene meetings and provide ongoing support and information to individual Home Visitors and Home Visitors’ support meetings Assess effectively the needs of the client through interview process and consultation with the referring agency, if appropriate. Identify client/carer needs to develop individual care plan to be provided by service plus any other external services both informal and formal supports. Plan to include strategies to work toward enabling client to be as independent as possible Communicate with client, identify and record client needs making appropriate referrals in accordance with HACC National Standards Respond to client requests for information about other services in the community which may be able to assist them; and with the client’s approval, make appropriate referrals for access to services. Develop and maintain a professional relationship with individual clients with the aim of assessing client needs and providing appropriate choices Provide appropriate support to primary carers, through information about services, individual support as required, and support primary carers in a group setting when numbers are sufficient and the need is indicated Respond to needs of carer by providing information, referrals. Support carer’s needs by providing social support for client and thereby providing carer with a break 2. Staff/worker support, supervision and recruitment Co-ordinate and supervise the recruitment of appropriate volunteers to ensure the effective provision of service to consumers. Support and supervise effectively a team of volunteers, ensuring continued skill and knowledge development to perform required tasks. Effectively recruit volunteers from a cross-section of the community, with the aim of ensuring adequate representation of the community in the volunteer group. Supervise and direct sessional workers undertaking roles within established social support groups to ensure all service and quality standards are being met and maintained Ensure an appropriate interview and selection process of volunteers, with the aim of recruiting individuals who meet the criteria of the work. Provide ongoing individual and group support to the volunteers of the service, ensuring that each volunteer is adequately supported in their role as a volunteer carer. Administrative and financial co-ordination, contract management and program accountability Efficient and timely co-ordination of the administrative and financial aspects of the program, including funding submissions, tenders and reporting Maintain effective working relationships with other VHV and HACC service providers locally and other broad Aged Care service networks Fulfil accountability requirements with the funding bodies and Springwood Neighbourhood Centre cooperative Ltd. Ensure the effective and efficient administration of the service to the HACC target group, relevant legislation and SNCC’s policies and procedures. Provide the required Coordinator’s report to the SNCC Board via the SNCC Executive Officer. Develop, maintain and continually review the administrative and information systems and policies to assist with the efficient operation of the service in accordance with Community Home Care Standards In consultation with the Executive Officer and Finance Coordinator, develop the service’s annual budget and monitor it on a regular basis, reporting as required. Operate the service within budget allocations and constraints Ensure monthly monitoring of the budget; discuss any issues with the Executive Officer. Ensure output targets are met and maintained Effectively promote the service within the community to ensure that the target group has an equitable access to the service Appropriately advertise the service in the community with aim of recruiting suitable volunteers to meet the needs of consumers and the service’s aims. Promote the service with other agencies and service providers to ensure that the aims and objectives of the service are understood, resulting in appropriate referrals Provide service according to the funding contract and guidelines of the funding body Provide service according to the policies and procedures of the SVHV and SNCC. Maintain service records including consumer details, volunteer details, statistical data, service documents and information. Consult with staff regarding service development, review and planning, ensuring staff input. Liaise with the funding body regarding funding and the development of the service; write submissions as required. Provide reports and data as required by the funding body and SNCC Executive Officer and/or Board. Participate in relevant training to ensure that the necessary skills are maintained to fulfil the requirements of the position. 4. Practice Standards Provide service according to the policies and procedures of the BMVHV services and SNCC. Maintain up to date knowledge of the Government’s service requirements. Service continuous improvement and quality Implement service structures to strengthen quality service delivery and outcomes. Engage actively in the development and review of all policies and procedures relevant to the VHV and social support programs and SNCC. Provide all volunteers with training opportunities to meet group and individual needs for skill and knowledge development. Ensure that all training opportunities provided are evaluated to determine their effectiveness. Consult with staff, clients and key stakeholders regarding service development, review and planning annually. Contribute to the development of service plans, aims and objectives on an annual basis, for public consultation. Undertake appropriate strategies to ensure client and service user input including annual client satisfaction surveys, annual volunteer satisfaction surveys and formal service feedback opportunities for clients and staff, to effectively evaluate the service, identify aims for service improvement and encourage consumer participation. Contribute to the development and review of service policies and procedures, in conjunction with the Executive Officer and SNCC Board. Contribute to the planning and review of the service in conjunction with the Executive Officer and the Board. Identify strategies for service development 6. Risk management and work safety Adhere to & comply with all SNCC & SVHV WHS policies, procedures and safe work practices Provide appropriate training and orientation re WHS practices, policies and procedures to all volunteers. Ensure staff are supplied with appropriate safety equipment e.g. portable current devices for SVHV volunteers. Ensure that you and any SVHV workers (paid and unpaid), under your supervision fulfil the required SNCC WHS staff and management responsibilities as per SNCC and SVHV WHS policies, including phone & home safety checks for all work activities to be carried out in a client’s home, compliance with risk management and risk assessment procedures. Promote and engender a safety and no blame culture within the SVHV service with clients, carers, volunteers and staff. Report any accidents, incidents or hazards promptly using the appropriate SNCC incident and accident and or hazard form. Team & Organisational Responsibilities Contribute to the development and cohesion of the broader staff team within the Cooperative, including paid and unpaid workers. Effectively contribute to SNCC staff meetings, providing positive input. Work collaboratively with other SNCC staff by sharing resources, skills, energy and ideas. Actively encourage access to, and utilisation of SNCC programs, services and facilities Prepare the SHVH annual report for inclusion in the SNCC Annual Report. Contribute towards and adhere to SNCC policies and procedure development & implementation. Skill Requirement Well-developed interpersonal and communication skills, both written and oral and the ability to communicate effectively with stakeholders. Ability to prioritise work tasks and apply initiative, and work effectively with minimum supervision. Ability to work both independently and as part of a team. Competent computer skills (Microsoft Word, Excel, email/internet, database) Experience requirements Experience working in the community sector Experience working with volunteers, providing support and training Experience administering a community service.
