IMPORTANT: Online Enrollment for the 2012-13 School Year
Get a jump on the new school year and save time on enrollment day. Begin the enrollment process
online at home or at one of several designated neighborhood sites.
At Home- Log in at www.mmsd.org/enroll. The website will be open on August 6th to begin your
2012-13 enrollment update. Complete the enrollment steps, print off your verification form and bring
it with you when you come to your child’s school to complete the process.
In your Neighborhood - Several neighborhood sites will offer pre-enrollment days, providing access
to a computer and people to help you with the online enrollment process. Then, when you go to your
child’s school in August be sure to bring your enrollment verification form.

NEW THIS YEAR – The MMSD will now offer an online payment option. You may pay your fees with
a credit card as part of the enrollment process.

Welcome
The Madison Metropolitan School District offers a convenient way for you to enroll your student via the web. While
you must still attend an enrollment session at school, completing the online enrollment step now will save you time! This
booklet provides information on what you can expect when you work in the online enrollment system and complete the
enrollment process at your student‟s school.
How to access the Online Enrollment System
Go to www.mmsd.org/enroll Once there, families can click on the Online Enrollment button and complete the forms, as
well as access the Infinite Campus Parent Portal. See the instructions listed on the enrollment page if you don‟t know or
have forgotten your Infinite Campus Password. The Infinite Campus Parent Portal provides an online communication tool
to help parents/guardians stay current with their students' progress and provides a portal to the Online Enrollment
system.
Questions you will be asked
You will be asked the following questions during the Online Enrollment process.
Will your student be 5 years old or older on or before September 1, 2012 and living in the Madison Metropolitan
School District?

Signature Page. You will be required to sign just a single form at the end of the enrollment process. This form
is known as the “signature page” and must be signed and returned during an online enrollment session to the
school where your student will be attending. Please call the District office for information if you are unable to
attend a session.
OPTIONAL FORMS
Food & Nutrition - Free and Reduced Lunch. Completing this online application makes it easy and convenient
to apply for mean benefits for your child(ren). Simply enter the requested information as accurately as possible, and all data will be instantly and securely sent to your child(ren)‟s school district for processing. Start
your family‟s meal benefits sooner by entering your information now!
Internal Transfer Request. Form and information for Madison Metropolitan School District residents requesting Internal Transfers. Both online and printable versions of this application in English and Spanish can be
found at https://infosvcweb.madison.k12.wi.us/internaltransfer For questions on internal transfers please call
the enrollment office at 663-4942 or 663-4952. Completed forms can be returned to your child‟s school or the
MMSD Enrollment Office, 545 W. Dayton Street, Madison, WI 53703 or faxed to 608-442-0660. Available in
English and Spanish.
Backup Field Trip Permission Slip. Allows staff from your child‟s school to take your child on community field
trips in the event that he or she fails to return a written permission for a specific trip.
Dual Language Immersion Application. The dual language program is open to all students, but preference is
given to families living in the Chavez, Glendale, Leopold, Midvale, Nuestro Mundo, and Sandburg attendance areas. All applications must be submitted to the Madison Metropolitan Enrollment Office at 545 W Dayton
Street, Madison, WI 53703. Applications may be dropped off in Room 205, e-mailed to Jeannie Retelle at
[email protected], faxed to (608)442-0660 or completed online at www.mmsd.org/DLIapplication. Application deadlines are the Friday following the third Monday in March in order to have a student entered in the

Q. When can I enroll my student online?
A. The Online Enrollment window opens in the beginning of August just prior to the start of a new school year. Check
the Online Enrollment website at www.mmsd.org/enroll for more information.
Q. Do I need to bring my student‟s immunization records with me on enrollment day?
A. Generally, parents do not need to bring any immunization records to enrollment. Families who are new to Wisconsin
can bring these records in to be copied, but in general the Madison Metropolitan School District is able to obtain these
records electronically from the state immunization database.

Student ID and school pictures will be taken during registration. MMSD has contracted Lifetouch to take all
school and ID photos. The ID photo will be taken separately from the school photo. You will then have a choice
of poses and packages for your student‟s 2012-13 school
photo. Watch for a mailing in August from Lifetouch,
which will include your school photo order form. Please
bring this order form and payment to registration. Students who are not available during registration week will
have their ID and school photo taken after school starts.
Madison Metro Bus Transportation
The Madison Transit System bus pass procedure will remain the same this fall. Semester passes and 10-ride bus
cards will be available for sale in the office and at registration. Checks for the semester bus passes should be
made payable to Madison Metro for $150 per semester*.
Checks for the 10-ride bus card should be made payable to
Jefferson Middle School for $10 per card*. These fees
are paid separately from registration fees. You may call
Madison Metro at 266-4466 or 267-8778 with any questions or concerns regarding the bus system.

First Day of Classes
6th grade students ONLY will report for their first day
of classes on Tuesday, September 4, 2012.
ALL students (6th, 7th & 8th) should report for classes on
Wednesday, September 5, 2012.
School starts promptly at 7:35 am with dismissal at
2:37 pm.
Buses will be operating on the first day of school. Bus
scheduled can be picked up during registration and they
will also be listed in the newspaper before school begins.
Please keep in mind these fees may change after this publication.
*If an ID card is lost or destroyed during the school year,
a $5.00 fee will be required to obtain another card.
**The Yearbook fee is optional. If you would like your student to receive a yearbook at the end of the school year,

In addition to the regular school fees, those renting a
school instrument for Band/Strings will pay a fee according
to the following proposed fee schedule:

The Health Information Update is part of Infinite Campus enrollment. Please make sure to update the
health information and to discuss with the school nurse any health concerns for your child.
Medication Administration Forms must be completed every school year for prescription or over-thecounter medications to be administered at school. There is a form for the parents‟ permission and a
form for the physician order. The parent must provide the medication in a labeled container. If a student needs to carry an Inhaler for self-administration, both forms must be completed and arrangements made with the School Nurse.
Primary Access for Kids is a program for students attending MMSD who are unable to get health insurance. If your student does not have health insurance, please contact Ms. Haney for information
about this program which allows your child to see a primary care provider for certain services at no
charge. This is not an insurance program.
Health Care Resources: You may contact the School Nurse for assistance in finding care. Whether it
is for glasses, an eye exam, a dental problem or emotional problems, there are resources in the community to help.
Field trips: If your student will need medication on a field trip, speak to the School Nurse in advance
so arrangements can be made.
In case of emergency or illness, it is vital that the school has a way to reach a parent. Please be sure
the school secretary has current phone numbers for home, work and/or cell for parents, guardians and
emergency contacts. Remember to update changes during the school year.

Dress for Success
There is a close relationship between student attire and student attitudes toward school. For that reason, the
district Student Conduct and Discipline Plan (#107) prohibits students from “dressing or grooming in a manner which
disrupts teaching or learning.” Please note the „particulars‟ as it relates to Jefferson students “dressing for success”.
These guidelines are similar to those in other of our middle schools.
