Page 16 Jefferson Jabber SCHOOL HEALTH OFFICE INFORMATION Health Office Staff at Jefferson: Tanya Haney, School Nurse 663-6440 Kristine Stilson, Nursing Assistant 663-6439 The Health Information Update is part of Infinite Campus enrollment. Please make sure to update the health information and to discuss with the school nurse any health concerns for your child. Medication Administration Forms must be completed every school year for prescription or over-thecounter medications to be administered at school. There is a form for the parents‟ permission and a form for the physician order. The parent must provide the medication in a labeled container. If a student needs to carry an Inhaler for self-administration, both forms must be completed and arrangements made with the School Nurse. Primary Access for Kids is a program for students attending MMSD who are unable to get health insurance. If your student does not have health insurance, please contact Ms. Haney for information about this program which allows your child to see a primary care provider for certain services at no charge. This is not an insurance program. Health Care Resources: You may contact the School Nurse for assistance in finding care. Whether it is for glasses, an eye exam, a dental problem or emotional problems, there are resources in the community to help. Field trips: If your student will need medication on a field trip, speak to the School Nurse in advance so arrangements can be made. In case of emergency or illness, it is vital that the school has a way to reach a parent. Please be sure the school secretary has current phone numbers for home, work and/or cell for parents, guardians and emergency contacts. Remember to update changes during the school year. For that reason, the district Student Conduct and Discipline Plan (#107) prohibits students from “dressing or grooming in a manner which disrupts teaching or learning.” Please note the „particulars‟ as it relates to Jefferson students “dressing for success”. These guidelines are similar to those in other of our middle schools. General – Not Allowed Coats or Jackets worn to class Bare feet Wallet chains or pant chains Sunglasses or „play glasses‟ House slippers Visible underwear Clothing that is too skimpy or revealing Clothing that carry profane, obscene, gang-related or offensive slogans or pictures. Hats, bandanas, hoods, hairnets, scarves, sweatbands, “do rags,” etc. Headgear worn for religious reasons is allowed. Tops – Not Allowed Muscle shirts Spaghetti straps, see-through mesh shirts, backless tops, halter tops, tube tops, tops that expose the stomach, and sleeveless shirts that expose the sides. Bottoms – Not Allowed Short shorts Sagging pants – even if you have gym shorts underneath! A student who wears a shirt that violates the dress code is asked to turn the shirt inside out or is offered a clean, replacement shirt. A student who wears sagging pants is offered a length of string as a belt or a belt. In some cases, a parent may be asked to bring replacement clothing to school. A student who repeatedly refuses to conform to the dress code may be subject to disciplinary action, up to and including suspension. Help your student Dress for Success!! Gammon Road Madison, WI 53717 Main Office: 663-6403 Safe Arrival Line: 663-6415 Fax: 442-2193 Thomas Jefferson Middle School was originally named Memorial Junior High. It was built in 1969 with the open classroom concept. Since then, the school has changed some, but the program of the school remains the same: As Principal Richard Meister puts it: “This is a child-centered-program rather than a subject-oriented approach.” Principal Richard Meister was the first principal of Thomas Jefferson Middle School. Have a safe and happy summer! See you in September!
