• Council struck the Innisfil Community
Health Committee (ICHC) in 2011
• 2012 – Innisfil Healthy Communities
Assessment Report (qualitative study
through York University)
• 2014…. Still need quantitative data
– Partnership with Georgian College
Research Analysis Program to conduct
survey

What are the needs of the community?
Survey objectives:
• Determine health status
• Find out how primary and other health
services are used
• What are the barriers to access health
care
• What are the health needs and services
required

Methodology
• 4 page paper survey
• Sent to random selection of 5,000
residents
• Responses collected between June
and July, 2014
• 17% response rate
