Sponsor a PADRES Family through our
Adopt – A – Family Program
The holidays are a magical time for children. For a child with cancer,
the holiday season is one that should be filled with joy and HOPE.
Please join us in making this time of year a special experience for a
PADRES family by sponsoring them or making a donation to PADRES
through our Adopt – A – Family Program.
Sponsoring a PADRES family is easy, just follow these simple steps:
1. Fill out the required forms, which include selecting the number
of family members you would ideally like to sponsor. Please
return via email or fax to Dora Bastida.

2. A PADRES Team Member will then contact you to match you up
with one of our families in need. We will make every effort to
try to match you with the appropriate family size and location
requested but may not be able to fulfill that based on families
in need. Most families PADRES works with are large. You can
however partner with other friends and colleagues to fulfill a
wish request.
3. To aid you in selecting gifts, we'll provide you with your family’s
wish list, which includes clothing sizes and interests.
4. Present the gifts to your sponsored family. Gifts must be
delivered in person or dropped off at the PADRES office.

Adopt-A-Family Program 2016 Participation Guidelines
Thank you for participating in the Adopt-A-Family Program! Please read the following Participation Guidelines to help make this another
joyous holiday season for PADRES children and their families.

DELIVERY OF GIFTS
To ensure a timely delivery, gifts must be delivered between December 1st – December 18th, 2016.
We encourage gifts be delivered directly to the family’s home if the family has approved home delivery (refer to the Family Information
Sheet). In order to find the best time to deliver your gifts, please call the family you are adopting at the number listed in the Family
Information Sheet. Families have been notified that there are no guarantees they will receive the exact or suggested gift items on their wish
list. Wish list includes additional members living in the household, gifts, are not required but recommended (i.e. grandparents).
REQUIREMENTS
 Please include a gift for each family member (i.e. a toy for a child, perfume for Mom, etc.) as well as a clothing item for each family
member (i.e. sweatshirt, jacket, sneakers, etc.).
 We recommend you provide a gift card to a grocery store convenient for the family (refer to the Family Information Sheet for
stores closest to the family).
 All gifts must be wrapped and labeled for each designated family member.
 All toys must be new and appropriate for children ages 0-18 (please no toy guns, knives, etc.).
 Monetary gifts will not be accepted. Please use gift cards instead of cash.
RECOMMENDATIONS
 Gift receipts are recommended to exchange for incorrect clothing and shoe sizes.
 For tax purposes, please keep copies of all gift receipts.
IF YOU ARE UNABLE TO DELIVER GIFTS
In the event you are not able to deliver gifts directly to the family’s home, please contact Dora Bastida at (888) 441-0056 x 703 or via e-mail
at [email protected] to schedule a delivery to the PADRES office during business hours. Our office is located at 303 N Glenoaks Blvd,
Suite 200 Burbank CA 91502. Our office hours are Monday-Friday, 9:00 AM – 5:00 PM. An appointment must be made before delivering

Agree to participate in PADRES Contra El Cáncer's 2016 Holiday Adopt-A-Family Program, an initiative benefiting
children with cancer and blood diseases and their families. By agreeing to participate, I acknowledge that I will fulfill
the family holiday sponsorship and that I will be matched with either a hematology (blood disease) or oncology
(cancer) patient and family members that are underserved and participants of PADRES Contra El Cáncer through
partner hospitals.
In addition, I acknowledge that gifts will be delivered to the home of my assigned consenting family or to the
PADRES Administrative office between December 1st – December 18th, 2016.
I also understand that families who provided a wish list will anticipate sponsorship of their family for the
holidays. If at any time, I can no longer fulfill my commitment as outlined above, I will immediately notify
PADRES Contra El Cáncer. Notification must be sent in writing via mail, fax or e-mail to PADRES Contra El Cáncer
no later than December 9, 2016, so that the family may be matched with another sponsor immediately.
