Could you represent patients and carers on
the Sustainability and Transformation Plan?
How can I get involved?
You can volunteer to be a patient or
carer representative on one of the STP
working groups. This means you will
help the groups think about service
changes and improvements from a
public perspective.
What commitment do I have to make?
We will expect you to attend meetings
in different parts of the county at least
once a month for the next six months.
You must be willing to share
information with other people in your
local community and feed back their
concerns and questions.
What support will I get?
We will pay your travel expenses and
brief you in advance of each meeting
to make sure you understand the
agenda, and feel informed and wellprepared.
What will my role be?
Your role is to ask questions
and challenge assumptions
from a patient’s or carer’s
perspective. You will help the
group to understand the
needs of patients and the
barriers they face.
You can draw on your own
personal experience, but you
should also feed back the
concerns and issues that you
are hearing from friends,
family members, neighbours
and other people in your
community.

What skills do I need?
You need to have an
interest in health and
social care services.
You may be interested
in a specific type of
health care, for
example, GP practices
or A&E services. You do
not need any specialist
knowledge as we will
support you before
each meeting.
