CEMETERY MEMORIAL AGREEMENT
FOR CEMETERY MONUMENTS, MARKERS, TABLETS, HEADSTONES, AND TOMBSTONES
This form is to be signed and submitted for approval to the St. Mary Parish Center Office
before your monument/marker/memorial is ordered and commissioned.
In keeping with the Rules and Regulations of St. Mary Queen of Creation Parish Cemetery we do hereby agree to the
following:
Any monument, marker, tablet, headstone or tombstone, hereafter referred to as Memorial, which we intend to place
on any grave at St. Mary’s Cemetery purchased by me/us, shall meet the following requirements:
• I/We understand your recommendation to wait a minimum of 4 months after burial to install foundations.
• All Memorials shall be installed on a concrete foundation, which cost is borne by the grave owner and, for which
the specifications of the foundation will be determined by the Memorial Supplier based on the size and weight
of the Memorial. Foundations must be installed by licensed contractors approved by Cemetery Management.
• Memorials shall be made of granite, marble or bronze.
• The Memorial type shall be (check one):

Family Monument, for a six grave configuration (3 by 2), which shall extend no more than 20 (twenty)
inches deep, by 90 (ninety) inches wide, by 48 (forty-eight) inches high, excluding the dimensions of
the base, which shall not be more than 10 inches high. Family Monuments are permitted in
designated areas only.
o
Other Monument which shall extend no more than 20 (twenty) inches deep, by 30 (thirty) inches wide,
by 30 (thirty) inches above ground, including the dimensions of the base, per single grave.
o
Marker which shall be flush to the ground and shall extend no more than 20 (twenty) inches deep, by 30
(thirty) inches wide, including the dimensions of the base, per single grave.

All carvings, art, icons, inscriptions, epitaphs, etc. must be of a religious nature and must be approved by the
cemetery management prior to order and installation. Names placed on memorials must conform exactly with
cemetery records.
It is recommended that a Latin cross appear somewhere on the memorial.
Interment of cremains is permitted in approved containers and in designated areas only, maximum of three
cremain burials per grave. Cremain Memorials must be flush with the ground, no more than 10 (ten) inches
deep by 16 (sixteen) inches wide.
Every occupied grave will have some sort of identification marker.
Only one above-ground memorial per grave is permitted in designated areas only.
All rules and regulations, including those regarding grave decorations, are to be strictly followed.
