In the spirit of caring for our neighbors,
360 Communities provides hope and support to people
by engaging communities to prevent violence, ensure
school success and promote long-term self-sufficiency.

From its inception, 360 Communities
recognized innovation and adaptability
would be essential in order to grow as an
organization and meet the dynamically
shifting needs of a demographically
changing community. 2012 was the
culmination of years of intentional work
evaluating our programs, researching
best practices and measuring outcomes
in the communities we serve. In 2012,
we restructured our leadership model
and realigned our programs to
position the agency for growth and
large-scale impacts.
In 2012, dedicated 360 Communities
staff and more than 1,100 volunteers
served more than 14,000 people and
documented measurable gains. New partnerships, new funding and community support has
positioned this organization for long-term sustainable growth. This speaks to what
360 Communities has been achieving for more than 40 years: connecting individuals to their
broader community, inspiring people to take ownership in writing their own success story
and establishing innovative collaborations to address the ever-shifting challenges we
all face.

360 Communities Partners For Success (PFS) family
support workers engage students, parents
and educators to build trusting relationships
that lead to school success and increased
parental involvement.
A safe home is the underpinning of a bright
future for any woman or child. That’s why
360 Communities has been committed
to violence prevention, intervention and
education since 1979.
During the 2012-2013 school year,
PFS staff worked in 27 schools in 7 districts and
provided crisis stabilization, learning plans and
communication between home and school with
722 families and 1,291 students.
In 2012, 360 Communities Lewis House
locations in Eagan and Hastings provided
safe haven and resources to 2,287 women
and children.

with three programs that fortify the bonds between parents,
students and educators.
[Kids are learning faster than ever before in
history and so whether the student is coming
to school not ready to learn or whether there’s
a crisis in their home causing instability that is
leaving them behind, when it’s time to catch
up that means they have to move faster than
kids in stable homes. So unless we wrap all
kinds of services around them, that’s not going
to happen ... So we need efforts like this.

Early Childhood Services
DA KOTA H E A LTH Y FA MI L I E S
360 Communities Dakota Healthy Families (DHF) works to promote
safe, healthy and nurturing homes for children and prepare them for
kindergarten. In 2012, DHF provided in-home visiting to 102 families.
C H I LD CA R E AWA R E
360 Communities Child Care Aware (CCA) partners with day care
providers to improve the quality of care and to teach about health and
safety. The program promotes early literacy, a foundation for school
readiness. In 2012, CCA provided support and consultation to 110
child care providers and family, friend and neighbor care providers.

Community Resources
Bridging a family or individual from hard times to self-sufficiency takes more than giving
food and financial relief. 360 Communities’ five food shelves and two family resource centers
are uniquely positioned to offer a network
of programs and support to address the
underlying barriers to success.
In 2012, 360 Communities’ network of five
food shelves and two family resource centers
served 10,959 individuals, connecting
families with food and emergency relief.
360 Communities values the more than
1,100 volunteers who gave of their time,
energy and talent helping serve their
neighbors through 11 programs in more
than 40 locations throughout Dakota County.
