Registration Process and Cancellation Policy
Registration for the 2017 Medical, Clinical Hypnotherapy Training is open from
Jan 15, 2017 and closes at 12am Feb 28, 2017.
Registration process is as follows:
Step 1- Download application
Step 2- Complete application, return via email or postal service
Step 3- Complete phone interview with Linda Gentry
Step 4- Upon acceptance in to the program
Payment in full of the registration fee is accepted between, March 12 through March 18,
2017 only. Only 6 students will be accepted in the class. The six accepted students will be
on a first come first serve bases during the payment dates listed above. Payment in full
must be made via Pay Pal online during the above dates. A Link to pay pal will be available
on the website only on the above dates
Step 5- Begin 1st weekend training as scheduled
Each application for the training program is reviewed by the instructor, Linda Gentry RN,
BS, CHt. Linda will contact you within two weeks of receipt of your application to schedule
your phone interview. You will receive notification of acceptance by March 05, 2017 at the
latest.
It is recommended you do not make travel or lodging plans until your acceptance in the
program.
THE CLASSES FILL UP QUICKLY - COMPLETE YOUR APPLICATION AS SOON AS POSSIBLE ONLY 6 STUDENTS ARE ACCEPTED

Cancellation Policy
Payment minus a $500.00 fee will be returned if cancellation is made before
March 18th 2017, 12am (midnight)
No returns after 12 am (midnight) March 18th 2017
Attendance is mandatory to all classes. If weather conditions are hazardous
class may be rescheduled for the safety of all participants
If the student has and emergency they should contact the instructor ASAP
If the class is cancelled by the instructor before the first day of class, March 25,
2017, 8 am. The registration fee is reimbursing 100% to the participant. The
instructor/MIH is not responsible for travel or lodging expenses due to the
cancellation.
