NOTE: If my application for credit is denied, my student will be given the opportunity to request additional Unsubsidized Loans
for which they might be eligible. If my application for credit is approved, I must complete and sign a valid PLUS Master
Promissory Note (MPN) at www.studentloans.gov, before WC will process the PLUS Loan. My student must be enrolled for a
minimum of six hours to receive loan disbursements. Additional assistance awarded may result in a reduction or cancellation of
loans. The borrower must sign a separate PLUS Loan Request Form and MPN for each dependent student. The parent who
completes this PLUS Request Form must also sign the Parent PLUS MPN
