the ground that the original license/registration document is lost or destroyed knowing that such original
license/registration document has not been lost or destroyed, I shall render myself liable to possible police
prosecution.

(a) The original Bank-in Slip made payable to the Hong Kong and Shanghai Banking Corporation
Limited for the account of "Airport Authority" account number 004-600-356133-001 quoting company
name as reference.
The fee schedule shall be referred to Part 5 of Airside Vehicle Licensing and Examination Handbook on
the Authority’s extranet;
