Thank you for allowing your child to create an online account with Wizards of the Coast and participate in Wizards’
organized play events. Our players are members of a global community bound together by their passion for digital and
table-top games such as Magic: The Gathering, Dungeons & Dragons, Duel Masters and Kaijudo. Please read this
parental consent form closely as it explains what information we collect from your child and how that information is
used, disclosed, and stored.
We are collecting your contact information in order to complete the account creation for your son or daughter and to obtain your consent for our information
practices. Any information collected from your child will only be used with your consent, which you may revoke at any time (please refer to the last paragraph of this
form on how to do this). Additionally, if you do not provide us with a completed consent form within seven days from the date your child created his or her account,
that account and any associated information will be deleted. When the account is created, she or he will be able to post to online forums relating to Wizards’ games
and compete in organized play events. Your child’s account will be subject to Wizards’ terms and conditions detailed here: http://company.wizards.com/tou. When
your child plays in an event, she or he acquires points which develop into a ranking in the worldwide community of his or her favorite game. Player ratings are stored
in organized play databases. We assign each player a DCI number (a unique number that is linked to their play history) which tracks play history and we rank players
from that play history in a variety of different ways including globally, nationally, regionally and by metropolitan area. Player rankings are publicly available through
our websites and include a player’s full name and country. We also report tournament results in online news articles on our websites; if your child participates in
organized play, you consent to their name and country appearing in online rankings and news articles.
The vast majority of organized play events are run by independent tournament organizers such as local store owners and game enthusiasts. To facilitate organized
play, Wizards provides these independent third parties with a number of tools including match-making and result reporting software. We also provide these third
parties with a “player” look-up tool that allows tournament organizers to identify interested players for upcoming events. In the event your child forgets his or her
DCI number (necessary to play in an event), a tournament organizer will be able to enter his or her name, city, state, or postal code into the player look-up tool to find
their DCI number. By having basic location information (city, state, and postal code) we can ensure that rankings remain consistent and accurate. If we notice an
anomaly in the ratings or other tournament information for an event your child participated in, we may contact him or her to verify information regarding that event.
We also provide players with information on upcoming product releases, tournaments and events. We do not sell player information, but we may share such
information with third party vendors who send marketing and administrative emails and conduct surveys on our behalf. If you do not want your son or daughter to
receive information about games, events or participate in surveys, please initial here __________ (this setting can also be changed in your child’s online account
settings). We retain the ability to contact you and your child with administrative information (e.g. new calculations for rankings, account information, updates to
terms and conditions, etc.).
Our organized play system is global and as such, information you provide to Wizards may be transferred across borders and from your country or jurisdiction to other
countries or jurisdictions around the world. By returning this form to Wizards, you expressly consent to the transfer of such information. The use of player
information is governed by our privacy policy, located here: http://company.wizards.com/privacy.
Parents and legal guardians may review personal information we have collected about their child, request deletion, revoke consent or refuse to allow further
collection or use of the information. To do so, please contact us at company.wizards.com/contactus.
I represent and warrant that the information provided below is true and accurate, and is provided for the purpose of consenting to my child’s account registration
and participation on Wizards’ websites and in its organized play system.
