This m atter w as opened to the New Jersey State Board cf
Dentistry on inquiry into the dental practice of Jeffrey Alan Banâ,
D .M .D . and Bernard Robert Rosenblatt, D .M .D ., as
related to in-
discriminate pu rchasing , p rescribing an d dispen sing ) as well as the
maintanalce
and inventory control of Controlled'Dangerous Substances;
the details of which are more fully described and set forth in an
investigative report prepared

John Faul Dizzia, Esq. Haggercy and
Drs. Band and Rosenblatt voluntarily and fully discussed
the various problems relating to Controlled Dangerous Substances
that they experienced in the conduct of their dental practice.
Through
counsel they also expressed a willingness to aetively
participate fn any rehabilitational and/or educational programs
administered or suggested by the New Jersey State Board of Dentfstry.
It appearing that the parties are desirous of resolvfng
this matter without further proceedings through the en try of the
within Order, and it further appearing that the entry of the within
Order shall adequately protect and serve the public interest and
for good cause shown ,

ORDERED
T M license of Dr . Jeffrey Alan Band to practice
dentistry in the State of New Jersey shall be and is hereby suspended
for a period of thirty (30) days coamencing on December l5, 1983.
The license of Dr. Bernard Robert Rosenblatt to practice
Dentistry in the State of New Jersey shall be and is hereby suspended
for a period of ninety (90) days cocmencing on December 1, 1983.
3 . Dr . Jeffrey A lan Band shall be and is hereby r sessed a G vil
penalty of two thousand five hundred dollars ($2,500.00). Dr.
Band shall forw ard to the office of the State Board of Dentistry
certified checks, money orders or attorney's trust fund checks in
the following amounts pursuant

assessed a civil penalty of seven thousand five hundred dollars
(17,500.00).
Rosenblatt shall forward to the office of the State
Board of Dentistry certified checks, money orders or attorney 's trust

shall be and are hereby assessed costs of investigation in the amounç
of four hundred forty-three dollars and sixty-eight cents ($443.68)
for which they shall be Joiptly and severally liable. Drs. Band and
Rosenblatt shall forward to the office of the State Board of Dentistey
a certified check , money order or attorney 's trust fund check in the
aforesaid amount within thirty (30) days of the effective date of
this Order .
Dr . Jeffrey Alan Band and Dr . Bernard Robert Rosenblatt
shall b0th enroll in and successfully complete the U .M .D.X.J . Dated
Physicians Program (Mini-Residency in Controlled Dangerous Substances
Prescribing) within one year
the effective date of this Order.

Dr . Jeffrey Alan Band and Dr. Bernard Rubelt xRosenblàtt
shall cease and desist from purchasing , prescribing , dispensing
and/or maintaining any and a11 Class 11 and Class IIN Controlled
Dangerous Substances for a period of two years from the effective
date of this Order .
