GENERAL PRICE LIST
These prices are effective as of January 15, 2012, but may be subject to change without notice.
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire.
However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other
requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the
statement we provide describing the funeral goods and services you selected.
Basic Services of the Funeral Director, Staff and Overhead:………………………………….. $ 2,695
This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is
already included in our charges for direct cremations, immediate burials, and forwarding or receiving of remains. The fee
includes the following:

Availability of our professional staff to respond to initial call and request for information/services 24 hours a day,
7 days a week, 365 days a year, including holidays.

Initial shelter and care of remains to include placement of identification documents on the remains and placement in
the cremation container, shipping container or casket.

Arrangement conferences, either in person, by telephone conference, e-mail or online to include interviews and
consultation with family members or other authorized agents during all phases of the arrangements to gather
information necessary for the coordination and timely execution of all details involved in the services/ceremonies
requested for the deceased.

Coordinating plans with the various parties involved including celebrants, cemeteries, churches, clergy, crematories,
fraternal organizations, military honor guards or any other parties involved to accomplish and provide the
services/ceremonies requested for the deceased.

Administrative assistance in the completion of necessary notices, authorizations, certificates and permits required for
the services/ceremonies requested for the deceased.

Overhead including utilities, facility maintenance, insurance and administration expenses. These costs are
associated with all the services we provide and are proportionately distributed among the various services to include
the common areas such as the parking lot, reception areas, restrooms and office space. Overhead includes
specialized funeral service equipment which is not seen by the public but necessary for us to provide services as
requested.

Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select
certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to
choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. It is the policy
of our firm to require embalming for any public visitation or ceremony.
Other Preparation of the Body:
Pennsylvania Rules and Regulations Number 13.201.6(i) requires that any human remains held 24 hours beyond death shall
be embalmed or sealed in a container that will not allow fumes or odors to escape, or be kept under refrigeration , if this does
not conflict with a religious belief or a medical examination. It is the policy of our firm to comply with Pennsylvania
rules and regulations that require the placement of human remains in a sealed container or be kept under
refrigeration if held 24 hours beyond death if those remains have not been embalmed.

This fee is for the initial transfer from the place of death to the funeral home. It represents the time required for qualified
personnel to make the transfer from the place of death. It also includes the vehicle used for the transfer and the necessary
equipment to comply with safety and health regulations.

This fee for our charges includes a proportionate share of the basic services of the funeral director, staff
and overhead; embalming; transportation/automotive equipment to include the initial transfer of remains
from place of death to the funeral home and transportation to local airport or other place of shipment
within a 20-mile radius and required shipping container (excludes shipping charges). The charge does
not include any attending rights, visitation or ceremonies and applies only to shipment within the
continental United States.

If you wish to arrange a direct cremation, an alternative container can be used. Alternative containers
encase the body and can be made of materials such as fiberboard or composition materials (with or
without an outside covering). The containers we provide are made of wood and fiberboard or cardboard.
This fee for our charges for a direct cremation (without ceremony) includes a proportionate share of the
basic services of the funeral director, staff and overhead; transportation and automotive equipment to
include the initial transfer from place of death to funeral home, local transportation to the crematory;
necessary authorizations; container and the crematory fees.

Social Security Administration
Social Security should be notified as soon as possible when a person dies. Our funeral home will file a
report of death with the Social Security Administration. You should get in touch with Social Security as
soon as you can to make sure the family receives all of the benefit to which it may be entitled. For more
information, write or visit any Social Security office, or phone the toll-free number, 1-800-772-1213.
You can speak to a representative weekdays from 7 a.m. to 7 p.m. You can also visit Social Security’s
Internet website: www.socialsecurity.gov.
Veterans Administration
Veterans discharged from active duty under conditions other than dishonorable and service members
who die while on active duty, active duty for training, or inactive duty training, as well as spouses and
dependent children of Veterans and active duty service members, may be eligible for VA burial and
memorial benefits. Our funeral home will assist you in completing the necessary application forms. For
more information, phone the toll free number, 1-800-827-1000. or visit the Veterans Administration
website at: www.cem.va.gov.
Military Funeral Honors
Upon request, the Department of Defense will provide military funeral honors consisting of folding and
presentation of the United States flag and the playing of “Taps” A funeral detail consists of two or more
uniformed members of the armed forces, with at least one member from the deceased’s branch of
service. If you want military funeral honors, our funeral home will assist you in obtaining the military
honors. You must provide a copy of the deceased’s DD214 (discharge papers) or proof of honorable
military service 48 hours prior to services to obtain military funeral honors.
Presidential Memorial Certificates
Presidential Memorial Certificates are issued upon request to recognize the United States military
service of honorably discharged deceased Veteran. Our funeral home will assist you in obtaining
Presidential Memorial Certificates by completing VA Form 40-0247. You must provide a copy of the
deceased’s DD214 or proof of honorable military service to obtain Presidential Memorial Certificates.
Payment Policy
Our funeral home has established a uniform payment policy to serve all families fairly and to prevent
misunderstandings. This payment policy enables us to contain our costs for all the families we serve.
Payment in full for our merchandise, services, and property is due at the time of completing the
arrangements or one full day before the service time or the use of our merchandise or property.
