Pursuant to Article 12, paragraph 5 of the Act on Medically Assisted Fertilisation (Official
Gazette 88/09), at the proposal of the National Commission for Medically Assisted
Fertilisation, the Minister of Health and Social Welfare hereby issues the
ORDINANCE
ON THE FORM CONFIRMING DISPOSAL OF REPRODUCTIVE
CELLS AFTER WITHDRAWAL OF CONSENT BY THE DONOR
Article 1
This Ordinance lays down the contents of the form confirming the disposal of reproductive
cells after the sperm cell or egg cell donor has withdrawn his/her consent.
Article 2
The confirmation referred to in Article 1 of this Ordinance shall be given in the confirmation
form annexed to this Ordinance and forming an integral part thereof.
Article 3
This Ordinance shall enter into force on the eighth day after the day of its publication in the
Official Gazette.
