ONLINE FORMS
If … your FAFSA (Free Application for Federal Student Aid) is missing:
Please visit www.fafsa.ed.gov and file the FAFSA as soon as possible. Designate Harvard as one
of your recipients by using “Harvard University, Cambridge, MA 02138.” Our federal code
number is E00468. If you have already filed your 2014 tax returns, you may be able to use the
Data Retrieval Tool to pre-populate the FAFSA for you. If you have already completed a FAFSA,
please call us at 617-495-1581 to confirm that we have your correct Social Security Number.
If … your PROFILE Data is missing:
Please visit www.collegeboard.org to complete the PROFILE or call 1-800-626-9795. The Harvard
College CSS code number is 3434. Once your CSS PROFILE has been processed (allow 3-5 days
for CSS to process), you will receive an email from the College Board’s Institutional
Documentation Service (IDOC) providing you with instructions on how to submit your tax forms
and any other supplemental forms that may be required.
If … your PROFILE Data is missing AND other documents are missing:
Start by following the instructions above, and then submit your documents to IDOC. Do
not attempt to submit documents before completing the PROFILE.
If … the Noncustodial PROFILE is missing:
Your noncustodial parent should complete a Noncustodial Parent's PROFILE online. You will
receive instructions from the College Board to send to your noncustodial parent explaining how
to complete this process after you have completed your custodial CSS PROFILE.

DOCUMENTS
If … tax returns or other documents are missing from your financial aid application:
Log in to your IDOC profile to upload/send your missing documents. (For US families, the 2014
Federal Tax Return refers to the 1040, 1040A, or 1040EZ form along with all pages and
schedules. For Canadian families, this refers to the T-1 General Form along with all pages and
schedules. For citizens of other countries, this refers to your national income tax return as filed
with your government, as opposed to the Notice of Assessment or summary of taxes paid. If a
national income tax return is not filed, see below.)
If … your parents are not required to file a 2014 tax return:
They should submit to IDOC a signed statement listing all sources and amounts of income for
their household in 2014, along with whatever documentation they do have, such as social
security statements, disability statements, wage statements, etc. You can download a non-filer
form for US Citizens or International Citizens from the College Board.
If … the Business/Farm Supplement is missing:
This form must be filled out for each business and/or farm in which the family has an interest.

If … a Cash Flow Statement is missing:
You can download the form from our website. Upload it via IDOC.
SPECIAL CASES
If … we have received a Paper Tax Return but your Parents’ 2014 Federal Income Tax Return is
missing:
Our office received paper tax returns via mail or fax. Tax returns must be sent/uploaded to
IDOC. Please upload/send your returns to IDOC.
If … Clarification of Assets, or Clarification of Income is missing:
Please call us at 617-495-1581 to clarify questions that we have about your financial aid
application.
If … the Parents’ 2014 Federal Income Tax Return is missing, BUT your returns have not yet
been filed:
Please call us at 617-495-1581 for further instructions. Please do not send draft taxes or 2013
taxes to IDOC.
