Boys/Girls 5th-8th Grade Soccer 2013-14 Permission Slip
Dear Parent:
Your child has expressed an interest in playing soccer on the school team. Please read the following
information carefully and complete the lower section.
Rules for Soccer - Important





Players must maintain a “C” average throughout the season.
Players must attend all practices and games unless arrangements
have been made with the coach.
Players must bring completed permission form before they will be
permitted to try-out for the team.
Players must have a completed sports physical on file before they
are permitted to try-out for the team.
Players trying-out must have paid their school registration fee.
Try-outs for the soccer team will last three days beginning Wednesday, August 7, and ending Friday,
August 9, 2013. Try-outs will start at 9:00 a.m. at the Mel Price Park and end at approximately
10:30 a.m. If your child has a soccer ball, please bring it to try-outs.
For players who make the soccer team, game and practice schedules for the season will be handed
out on Friday, August 9. When school begins, practices will take place after school (about 3:30) and
last until 5 p.m. Practices after school- some students walk, ride bikes, or have someone drop them
off. If they arrive anytime between 3:00 and 3:30, that will be fine.
Players that make the team will need the following equipment:


Cleats
Shin guards
Parental support is always an asset to team sports. If you are able to
come out and cheer us on at the games – please do! Thank you for your cooperation.
