Close Out Meeting & Post On-scene Activities 1 Objectives • Participate in the on-scene portion of an accident • Explain your role on a working group • Explain your role as a party spokesperson • Describe a submission to the Board • Understand the objectives of a Public Hearing • Understand the NTSB accident report process 2 Close Out Meeting • Call the meeting to order • Identify emergency evacuation for meeting room • Identify CPR qualified individuals • Specify individual to call 911 and backup • Excuse all members of the news media, attorneys, insurers, persons representing claimants, and others not authorized to participate 3 Close Out Meeting – Field Notes • Final on-scene meeting • Each of the groups should have reached agreement on the technical accuracy of the field notes • Each of the group members should have a copy • Each party spokesperson/representative should have a copy of the field notes for each group 4 Close Out Meeting • Field notes are preliminary and the investigation is not complete • The restrictions regarding release of information continues 5 Post On-scene Activities Continued contact with: • Spokespersons • Members of the working groups If formal testing or laboratory work is performed the parties will normally: • Have input with the testing protocols • Witness the actual testing A draft group factual report will be sent to the working group members to verify factual accuracy 6 Technical Reviews The Investigator-In-Charge prepares a draft factual report or assembles all group chair factual reports The party spokespersons will be asked to review and check for: • Technical accuracy • Factual accuracy 7 Post On-scene Activities Formal follow-up actions • Sworn Testimony • Investigative Hearing - Decided by the Board with input from parties - May have new parties added (manufacturers, product specialists, etc.) - List of witness’s and exhibits predetermined 8 Post On-scene Activities Public Hearing CFR 49 845.2 Nature of hearing. • Convened to assist the Board in: - Determining probable cause and reporting: Facts, conditions, and circumstances of the accident • Ascertain measures which will tend to prevent accidents and promote transportation safety • Fact finding only 9 Post On-scene Activities Investigative Hearing Format - Three questioning groups: • Board of Inquiry (NTSB) • Technical Panel (NTSB) • Party Representatives (on-scene and/or upper management) Witnesses - Selected by NTSB may have Party input - Have information related to accident or might aid in preventing the accident - Most exhibits pre-determined 10 Post On-scene Activities Party Submissions • Factual Report • Analysis • Findings • Probable Cause, and; • Possible Recommendations Submit to IIC and to the other spokespersons IIC forwards to the Board and enters in the docket 11 Internal Post On-scene Activities Office Director’s draft is circulated and reviewed by: • General Council • Recommendations • Research and Engineering • Public Affairs • Managing Director • Governmental Affairs These individuals will possess the party submissions 12 Safety Board Sunshine Meeting • Sunshine Act requires a public Board meeting • Staff presents the draft report • You will be invited to attend that meeting if one is scheduled (also webcast) • Only Board members and their staff participate in the discussion • The Safety Board adopts, modifies or rejects the draft report 13 Final Report A majority vote by the Safety Board to adopt the report will determine: • the probable cause of the accident • the findings and conclusions • and the recommendations to prevent recurrence of a future accident Once approved it will be made available electronically at NTSB.gov Recommendations will be delivered to the recipients by fax and/or email and mail 14 15
