You may have seen a sentence such as this “proving” you have an amazing brain if you can read
it. But we don’t want you to play word games in order to figure out how to submit your data!
To submit data into the QualityNet web-based measure tool, you will need:
 An active Security Administrator for the QualityNet Secure Portal or a Basic User with
the data entry role.
o If you don’t have anyone in these roles, start now—the process takes
approximately four (4) weeks!
 Data collected from your encounters from 2015:
o ASC-6 (Safe Surgery Checklist Use),
o ASC-7 (ASC Facility Volume Data on Selected ASC Surgical Procedures),
o ASC-9 (Endoscopy/Polyp Surveillance: Appropriate Follow-Up Interval for
Normal Colonoscopy in Average Risk Patients), and
o ASC-10 (Endoscopy/Polyp Surveillance: Colonoscopy Interval for Patients with a
History of Adenomatous Polyps – Avoidance of Inappropriate Use).
 Data entered for ALL of the above measures. In the event you have zero cases, enter “0”
for that measure.
The deadline for submitting these data is AUGUST 15, 2016.
