Congratulations! Your school has successfully completed Olive Garden’s Pasta for Pennies program to benefit The Leukemia &
Lymphoma Society. You have helped raise vital dollars needed to find a cure!
Please complete the following information so that we can arrange the Hospitaliano!
Lunch for the ONE top fundraising classroom in your school (max. 30 people).
THIS FORM MUST BE RETURNED WITHIN
TWO WEEKS OF YOUR CAMPAIGN’S COMPLETION ALONG WITH WRAP UP FORMS to be eligible for all incentives.
Donation of $750 or more MUST be received in order for LLS to make reservation with Olive Garden.
** DO NOT contact Olive Garden managers directly. LLS Staff will schedule all lunches.
**Please allow at least ten days for Student Series Staff to scheduling your pasta lunch.
***Each Campus will ONLY be allotted a ONE TIME reschedule with Olive Garden.
(Please, be mindful of School Functions, Assemblies, Field Trips, etc. when selecting dates)

Please give us three possible dates and times for lunch Pick Up/Dine-In** (if applicable). Lunches are best scheduled for pickup at your local Olive Garden restaurant Monday through Thursday after 11 a.m. when the restaurant opens.
**Dine-In w/kitchen Tour best scheduled at 10am (tour at 10:30am and meal served at 11am).
Any campus staff member or parent volunteer may pick up the lunch from Olive Garden.
