Job Summary:
The family navigator of Sabathani Family Resources Homeless Prevention program will provide
community outreach, assessment, and case management to assist families with maintaining
permanent housing solutions.
The Homeless Prevention Program provides rental assistance utility assistance, and supportive
services directly related to the prevention of homelessness to eligible families who are in
danger of eviction, foreclosure, or homelessness. The program is designed to stabilize families
in their existing homes or find more affordable housing.
Core hours for this position are Monday through Friday from 8 to 4 pm

Core Job Functions:
Duties and Responsibilities:






Provides assessment, case management, evaluation, and relocation services. Assessments focus
on families’ strengths and barriers including employment, behavioral health, criminal history,
and substance use and refer households to appropriate levels of housing and rental assistance.
Assist families with establishing and meeting housing goals and identifying appropriate steps
towards achieving goals, including modifying goals as needed and evaluation of progress.
Maintain collaborations with Housing Link and other community resources serving families at
risk for homelessness
Maintain a precise and accurate documentation of caseload and case management services.
Maintains participant files, statistics, and weekly reports.
Cultivate relationships with landlords and property management companies serving low income
families.
Set and observe appropriate boundaries with clients; observe client confidentiality

Position Requirements:
Qualifications, Skills, Abilities:









Bilingual, Bicultural (English/Spanish)
Experience in case management
Knowledge and experience with local housing laws, regulations, and available community
resources
Knowledge of community resources and services serving the Latino community in Minneapolis
Ability to work with diverse communities. Experience and knowledge with the challenges of and
resources for families, survivors of domestic violence, and transitioning youth
Excellent written and verbal communication (Spanish/English), culturally competent people skills
Computer literate – experience with MS Office: Word, Excel, Outlook, creating publications, and
using the internet
Ability to identify key issues in problem solving and implement effective and creative solutions
Ability to work under pressure, address multiple priorities and meet deadlines
Required Education and Experience
Bachelor’s Degree in relevant field required. Minimum of 3-5 years’ experience working on self-reliance
in a social service agency. Track record of assisting clients in a social service agency. Superior
communication (written and oral), planning and organizational skills. Ability to multi-task in a fast-paced
and changing environment. Strong understanding of working with individuals of differing ages, races,
and socio-economic backgrounds.

At a minimum serve 110 individuals monthly,

Attends 80% of Dept. and Agency monthly

policies
