GENERAL PRICE LIST
These prices are effective as of March 1, 2016. Prices are subject to change without notice.
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However,
any funeral arrangements you select will include a charge for our basic service and overhead. If legal or other requirements mean you
must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral
goods and services you selected.

This charge for our basic services and overhead will be added to the total cost of the cremation arrangements you select. This fee is
already included in our charges for direct cremations. This charge includes, but is not limited to, availability of staff 24-hours a day, staff
and facilities to respond to initial request for services, consultation with responsible party to determine services desired, coordinating
service plans with cemetery, crematory and/or other parties involved in final disposition of the deceased, confirming identification of the
deceased with a positive identification system, and filing of local death certificate.

THE STANDARD NEPTUNE PLAN – $1,899.00
The Neptune Plan includes the Neptune Preneed Direct Cremation Package (preneed) and the
Neptune Memento Package (retail).
NEPTUNE PRENEED DIRECT CREMATION PACKAGE
This package is sold on a preneed basis.
This package includes the basic services of the funeral director and staff, transfer of the body from the place of
death within Seller’s Service Area (75-mile radius of Beneficiary’s residence as listed on the Agreement) to a
licensed climate controlled holding facility and to crematory facility, with use of a positive identification system, filing
of death certificate, the actual process of cremation with cardboard cremation receptacle, disposition of the
cremated remains by common scattering at sea or return to person designated within the Authorization for Cremation
and Disposition.
NEPTUNE MEMENTO PACKAGE
This package is sold on a retail basis and is not included on any Preneed Funeral Agreement.
This package includes high-gloss memento chest with latch, cherry finish or biodegradable urn (autumn leaves), cherry
finish photo keepsake, 25 thank you cards and Neptune information book (delivered by express mail or courier). TRANSPORTATION AND RELOCATION PROTECTION PLAN – $499.00
This plan is only offered at the time of prearrangement.
This plan protects the Beneficiary of the Preneed Funeral Agreement from incurring additional out-of-pocket
expenses if death occurs while Beneficiary is traveling anywhere in the world or if Beneficiary relocates within the
continental United States. (Please ask for further details.)
*No refund or credit will be issued for package sale goods or services which remain unused by the customer at the
time of need.
The Standard Neptune Plan represents a total savings of $1,288.00 over itemized General Price List.

PRIVACY NOTICE
Maintaining your trust and confidence is at the core of our
business. We value the privacy of the information you have
entrusted to us concerning the funeral, cremation, or cemetery
services we provide to you and your family. This privacy notice is
being sent to you on behalf of one or more subsidiaries of
Service Corporation International (“SCI”) (collectively referred to
herein as “we” and “affiliates” of each other) that offer pre-need or
at-need financing (either directly or as an insurance agent, as
described below). Although these SCI subsidiaries operate as
separate companies, the treatment of nonpublic information by
each of them is consistent with this notice.
In the process of providing financial products and services to you,
such as advance (pre-need) trust arrangements or time-of-death
financing plans, it is necessary to collect and maintain nonpublic
personal information about you, such as your Social Security
number, name, address, transaction history, bank account
information, and credit-related information such as credit scores.
We collect this information when you apply for financing, from
any contract with us that you sign, from processing or
administering your financial transactions, and, occasionally, from
credit reporting or collection agencies.
In addition, we sometimes act as insurance agents for a third-party
insurance company in connection with advance (pre-need)
insurance-funded arrangements. In connection with those services,
we may collect information requested by the third-party insurance
company, such as (i) Social Security number, name, address, email
address, date of birth, phone number, pertinent health condition
and medical history, and details of any other life insurance held by
the insured individual; (ii) Social Security number, bank account
information, credit card information, name, address, and email
address associated with the person paying for the insurance; and
(iii) beneficiary designations. We forward that information to the
third-party insurance company along with information concerning
the related arrangements that you have made with us.
We sometimes serve as a re-seller of (pre-need) transportation and
relocation plans, and we may collect contact and payment card
information in connection with the purchase of such a plan, as
well as the name, address, date of birth, and phone number of the
beneficiary. We forward some of that information to the thirdparty transportation and relocation plan provider, along with
information concerning the related arrangements that you have
made with us. Consistent with our respect for your privacy:
 We will not disclose nonpublic personal information to any
affiliates or non-affiliated third parties except as described herein,
or as permitted by law (e.g., to provide financial, funeral, or other
services requested or authorized by you, or to respond to a judicial
or administrative order or subpoena, or protect against fraud).
 We may share nonpublic personal information we collect about
your transactions and experiences (but not information about your
creditworthiness) for everyday business use and marketing
purposes by our affiliates who offer funeral, cremation, and
cemetery-related products and services. We may share nonpublic
personal information with service providers who provide support
services or marketing services on our behalf. We also may share
nonpublic personal information with financial institutions with
whom we have joint marketing agreements with respect to the
joint offering, endorsement, or sponsorship of financial products
and services. We require any such affiliates, service providers, and
financial institutions to refrain from using or disclosing any
nonpublic personal information for any purpose other than the one
for which it was received.
 Within the SCI family of companies, we limit access to
nonpublic personal information to those employees and
representatives who need to know such information in order to
serve you. We maintain physical, electronic, and procedural
safeguards to protect the security of your nonpublic personal
information as required by law.
 When we believe you may be interested, we may also share
nonpublic personal information with certain companies that are
not affiliates of SCI (e.g., floral providers or companies in the
senior living industry), so that those companies can market their
own products and services to you.
You may prohibit or “opt out” of certain of the sharing of your
nonpublic personal information described above. In particular, you
may opt out of our sharing of information concerning you with
non-affiliated companies or with our affiliates for such nonaffiliated companies’ and affiliates’ marketing purposes. To do so,
you should either call us (toll free) at 1-866-805-9617 or email us
at: [email protected]. We will refrain from sharing your
information with non-affiliates or with our affiliates for their
marketing purposes for thirty (30) days, in order to allow you to
opt out of such sharing.
If you limit sharing or disclosure of information for an account
you hold jointly with someone else the limitations you select will
apply to all persons on the account unless you direct otherwise,
using one of the methods set forth in the preceding paragraph.
Please note that, in the event you request that we obtain financial
or other products and services for you from a third party (such as
an insurance company or a transportation and relocation plan
provider), such third party’s use of your information will be
subject to the third party’s privacy policy.
