Completion Instructions
Below you will find step-by-step instructions for completing the Training Provider Information Form. If
you need any help, please call the Educational Institution Line at the Learner Income Support Office.
The Learner Income Support Office processes funding applications based on the academic year, which
typically runs from August 1 to July 31 the following year. Enter all sessions the learner is attending
within an academic year. If there is a break longer than three weeks between sessions, please complete
a new funding application and Training Provider Information Form. A funding application can only
include session dates which have a start date within the same academic year.
If you have completed and sent the Training Provider Information Form to the Learner Income Support
Office and there is a change in session dates or costs, please complete and submit a new Training
Provider Information Form. Please mark “Revised” on the form.
In some situations, a new Learner Income Support Application or a Change in Circumstances for Learners
form is required e.g. a change to a session date that is greater than 30 days, or a request for additional
time to complete a training session. A new Training Provider Information form is required.
Social Insurance Number
Enter the learner’s Social Insurance Number. The Social Insurance Number is a nine-digit number that is
required to work in Canada or to receive government benefits.
First Name
Enter the first name of the learner.
Last Name
Enter the last name or surname of the learner.
Learner Status
Enter whether or not the learner is attending an approved full-time or part-time program.
Full-Time:
Full-time learner is a person who is enrolled in the minimum weekly hours of training for the program as
follows:

Basic Skills and English as a Second Language programs – minimum 20 hours per week;

Academic Upgrading programs at the grade 10 to 12 level – minimum 18 hours per
week;

Integrated Training programs – minimum 25 hours per week;

Occupational Training programs – minimum 25 hours per week.
Part-Time:
An approved training program with fewer than the minimum weekly hours required for a full-time
learner.
Training Provider Name
Enter the name of the training provider.
Institution Code
Please enter the provincial institution code of the training provider. If you don’t know your provincial
institution code it can be found within the Alberta Works Policy Manual under Tools and Resources.
Campus Name
Enter the name of the campus where the learner is attending training (if learner is not attending the
main campus).
Campus Location
Enter the city/town where the campus the learner is attending training is located.
Program Name
Enter the name of the program that the learner is attending.
Program Code
Enter the program code that has been assigned to the program. This code has been assigned to the
program through the program approval process with the Government of Alberta. A list of program codes
can be found within the Alberta Works Policy Manual under Tools and Resources.
Tuition (paid to training provider on behalf of learner)
Enter the amount of money charged for teaching or instruction for each session the learner is attending.
This is the actual tuition cost for the learner.
Mandatory Fees (paid to training provider on behalf of learner)
Enter the amount for mandatory fees for each session the learner is attending. Mandatory fees are flatrate charges incurred by all learners in an approved training program for libraries, technology,
laboratories and any other facilities for which the use of is necessary to both assist in instruction and
program completion, but does not include tuition fees and fees for health and dental benefits.

Student Association Fees (paid to training provider on behalf of learner)
Enter the amount for Student Association fees at the training provider for each session the learner is
attending. Student Association fees means the fees that are payable to a student association body, for
student association activities and does not include health or dental benefits.
Books (paid to learner)
Enter the cost of books for the session the learner is attending.
Supplies (paid to learner)
Enter the cost of supplies for the session the learner is attending.
Number of Courses
Enter the number of courses the learner will be attending for each session (if applicable).
Session Start Date
Enter the session start date for the learner’s program. “Session” is a term used to define the length of
time the learner will be attending the program without a designated break. It is synonymous with
academic term or semester. The start date must be within the current academic year.
Session End Date
Enter the session end date for the learner’s program.
Name of Training Provider Official
Print or type the name of the training provider where the learner will be attending the program.
Title of Training Provider Official
Print or type the name of the staff member at the training provider that is authorized to complete this
form.
Telephone
Enter the telephone number of the staff member completing the form.
Email Address
Enter the email address of the staff member completing the form.
Training Provider Official Signature (in Ink)
The person who has completed the form must sign the form in ink.
Training Provider Official Stamp (if available)
Stamp the form with the training provider’s official stamp (if available).
