Instructions for Numerator/Denominator Data Reporting for Cohort 1 This document explains how to submit numerators and denominators to DARTNet. The following tasks must be completed before you submit numerator/denominator data: Complete the DQIP with your practice’s selected data reporting method Schedule a data reporting meeting with your CHITA Obtain a login for and be familiar with DARTNet’s Practice Performance Registry A note about reporting periods: Each reporting period captures the previous 12 months from the quarter end looking back. For the Baseline data submission, each metric requires submission for each measure’s set of numerators/ denominators. After the Baseline submission, you will only need to report one set of numerators and denominators for each metric, corresponding to the 12-month reporting period that just ended. The figure below shows the reporting measurement periods. DARTNet’s Practice Performance Registry (PPR) – Login and enter the data directly into the site. 2. Excel spreadsheet – Using a secure cloud server called Egnyte, you download a file, enter your data into the file, then resubmit it through Egnyte. Page 1 of 4 Option 1. Using DARTNet’s Practice Performance Registry (PPR) to Report Numerator/Denominator Data 1. Go to: https://sharepoint.dartnet.info/ When prompted for a username, please enter DARTNET\<enter your username>. 2. On DARTNet Members homepage under Home header to the left, select “Numerators & Denominators.” 3. Click “+ new item” to enter the practice’s data for the first time a. Once you have reported Numerators/Denominators, you will locate your practice in the list by Practice ID. b. Select the “Edit” icon to enter next quarter’s data. 4. Practice: Choose your practice from the list (will link to ‘Practice ID’ in PPR). You may also type your Practice ID in the box and click the button or hit the Enter key. 5. Provider: Optionally, enter the provider name (Last, First). This field is available for practices where numerators and denominators are reported at the provider level. 6. Metric: Choose which measure (metric) you will enter one at a time. You may also type the Metric Name in the box and click the button, or hit the Enter key. If you don’t type the exact name, a list of choices will be offered. 7. Quarter End (Reporting Period): Choose Calendar Quarter and reporting period for the metric you are entering. You may also type the Quarter name in the box (follow the convention of “Q1<space>FY<space>2016”), and click the button or hit the Enter key. Choose the Quarter End / Measurement Period for the metric you are entering. For example, Q4 FY 2015 = 2015/01/01 – 2015/12/31, indicating quarter end captures 12 full months of data. **ENSW reporting periods are each rolling 12-months** 8. Numerator/Denominator: Type each number. 9. Save: Click “Save” button after entering data to submit it. 10. Confirm on the “Numerators & Denominators” page that your entry is listed in the entry row by Practice ID. Page 2 of 4 Option 2. Using Excel to Report Numerator/Denominator Data For those practices that want to bulk enter numerators and denominators for each quarter using Excel, please follow these directions. You will obtain a template file from DARTNet and then resubmit the completed file using Egnyte. Egnyte is an encrypted, HIPAA compliant, cloud-based data transfer system. Specific Egnyte folders will be established for each data-providing entity. These folders can be established at the individual practice level or at the organizational level at the request of the clinical organization. For organizations providing numerator and denominator data for a set of practices we are happy to do this through one folder using one Excel workbook or divide this up by individual practice. Practice users will receive temporary access rights to their specific Egnyte file folder established by DARTNet for file transfers. Obtaining the Excel file: 1. Send an email to [email protected] indicating that the site is ready to report data. DARTNet staff will send a password protected time-limited link to the site’s personal Egnyte folder. The password will be in one email and the link in another email. The link will be good for 48 hours. Do not alter the data in the “shaded” columns E-L, as they contain read-only metadata. 2. Click the link, enter the password, and pick up the file. Save it to your computer in a secure location. 3. Enter today’s date in the “Today’s Date” column. 4. Enter the Numerator / Denominator as a whole number in each column. 5. Optionally, enter the provider “Last, First” name. This column is available for practices where numerator and denominators are available at the provider level. If you are reporting at the practice level, as opposed to the provider level, ignore the “Provider” column and add the metrics rolled-up in each respective group. Note: an estimated number of rows per provider per Quarter and Metric have been generated. Page 3 of 4 If you need to create additional rows for additional providers, copy and paste the relevant Quarter/Metric row as a new row. If there are too many rows for the number of providers, please leave the extra rows’ columns A-D blank. Submitting the Competed Excel file back to DARTNet: 1. After populating your data into the Excel template, save the file back to Egnyte. If this is done within the 48 hour window, then the same link and password will work. Click the link, enter the password, and place the back in the secure folder. DARTNet will be automatically notified when this occurs and we will pick and erase the file. 2. If the original file is not picked up or replaced within the 48 hour window, email [email protected] to request a new link and new password. Each password will only work with the specific link for 48 hours. This completes the submission of numerator/denominator data.
