Electronic Spanish Questionnaire Directions
To the student: Please take this document to the Spanish teacher you will have for your junior year at
your sending school. This questionnaire plays an important role in helping you to continue in your
Spanish language study. Without this information, we will have a difficult time designing a Spanish class
for you. If you do not yet know who your Spanish teacher will be, ask your current teacher or language
department head to complete the questionnaire.
To the Spanish teacher: As you may know, your student has decided to spend one semester of his/her
junior year at the High Mountain Institute. We will enroll him/her in the appropriate Spanish class based
on the information you share with us and on the student's completion of a placement test.
Our primary goal is to help our students continue their study of Spanish and re-integrate successfully into
sending school classes after their semesters here. With up to 48 students from different sending schools
around the country, it is helpful for us to know what each student needs during his/her semester with us.
Please include a course syllabus and any additional information you think might be helpful.
Thank you very much for taking the time to help us meet the needs of your student(s). If you have
questions or concerns, please contact Aimee Goldstein, Administrative Assistant for the HMI Semester, at
[email protected] or 719-486-8200 x106.
You will need the following information to complete the online questionnaire:

Student name_______________________________

Title of Spanish class for junior year (2017-2018)

2017-2018 Spanish teacher’s name

Spanish teacher's email address and school phone number
To complete the online questionnaire, please go to
http://www.hminet.org/accepted
and click on the link for the Spanish Questionnaire.
The questionnaire should take about 20 minutes to complete. It will primarily ask questions
about what material the student's Spanish course will be covering while he or she is away.
This information helps us prepare the student for subsequent Spanish courses.
Thank you!
The electronic form is due by May 15 for Fall Semester students and October 1 for Spring
Semester students.
Please do not send us this sheet!
