Subject Selection Checklist
Remember to:











Choose your classes wisely after thoughtful consideration.
Share this information with your parents, and include them in the
decision making process.
Print clearly in order to avoid errors.
Include a valid phone number we can call if we have any questions.
Obtain teacher signatures for all Honors or Advanced classes.
Complete the entire form.
Choose your electives in priority order. Your Academy Course is
your first choice; the second request is your second choice, etc. It is
important that you fill in all the requests. We will try to honor your
first two choices.
Do your best on the FCAT and End of Year Exams. The results will
determine your placement in elective courses. For example,
Intensive Reading and Intensive Math will be substituted for your
electives if you receive scores of 1 or 2.
Choose a Florida Virtual School (FLVS) class only if you plan to enroll
in an online class. These classes are offered in a school lab with a
virtual teacher. Students will be expected to remain in the virtual
lab all year long.
Sign and date all forms.
Submit any course changes by June 2nd, 2014. No changes can be
made after that date.
