Brothers,
Here is your template for the Harm Reduction Report. Below you will find 5 sections with a number of
questions related to different aspects of best harm reduction practices. Please go through and answer all
questions that you find in the following sections. Answer each question as fully and as detailed as possible.
This is not a time to hold out on anything that you have done. Remember that this is the opportunity to
talk about all the things that you have done over this past semester, not what you plan to do in the future.
Once you have completed the report, please send a copy to Headquarters. The report is due back to
Headquarters by Decemeber 1st or May 1st.
There are three ways to submit your report:

directly affect your Risk Management Assessment for the upcoming semester. If you have any questions,

SUBMITTING GUIDELINES
Upon completion of this report, save this document and email it to
[email protected] by the appropriate deadline. After a report has been submitted,
the submitting officer should receive a notification within 48 hours, confirming the report has
been accepted by the Office of Harm Reduction. If a receipt email is not received after the
report is submitted, please contact the Office of Harm Reduction.
In an effort to ensure accuracy in reviewing each chapter’s supporting documentation for
the Harm Reduction Report, all supporting documentation must be combined into a single
PDF or Microsoft Word File to be eligible for review. Simply put, each chapter should be
submitting two documents, the completed Harm Reduction Report and a document containing
all supporting documentation.

You will have two options when submitting the report. The first option is to send these documents
electronically to [email protected]. After you have sent both documents to this address you will
receive an email, confirming their receipt, within 48 hours. If a confirmation email is not received within
this timeframe, please contact the Office of Harm Reduction. The second option is to send a physical copy
of these documents to Lambda Chi Alpha International Headquarters, 11711 N. Pennsylvania Street, Ste.
250, Carmel, IN 46032. We look forward to reviewing the chapter’s efforts over the past semester, and
please feel free to contact our office with any additional questions.
