NIC EXAMINATION OVERVIEW DVD ORDER FORM
The NIC Examination Overview DVD contains an informative introduction into NIC testing and
presentations on Cosmetology, Nail Tech/Manicuring, Esthetics, and Barber Practical
examinations. The DVD does NOT contain demonstrations of tasks addressed on the practical
examinations.
Please complete the Order Form and include a phone number and/or E mail address. This
information is requested so that in the event of any questions about your order a NIC
representative may contact you prior to shipping your order. The cost of the NIC Examination
Overview DVD is $25.00 (includes shipping). Be sure to indicate the number of DVD’s and
enclose the total fee. Checks or Money Orders should be made payable to:

NIC IS NOT ABLE TO PROCESS PURCHASE ORDERS. ALL ORDERS MUST BE

receive the DVD prior to the date you need to receive the DVD. All DVD
orders are processed within 5-7 days of receipt. NIC will not authorize any
refunds due to late receipt of a DVD.
