Welcome and Opening
Steve Russell called the meeting to order at 9:06 a.m.

Student Representative was not present.
IV.
Approval of Minutes

September 12, 2014 minutes
Motion by Sue Pruit, second by Vern Ulrich, to accept the minutes; the motion
carried unanimously.
V.
Site Reports
1.
Lahontan – Nothing to report.
2.
Numa
Mr. Purrell stated that the new blacktop became slick from the recent moisture
causing issues with ice. One staff member fell getting out of her vehicle, but was
not injured and did not seek medical treatment. Mr. Purrell received a parent
complaint yesterday from a mother who stated that both of her children slipped on
the blacktop after school. Rain gutters are an issue, as the dripping creates small
puddles that turn to ice which does not melt by the end of the day. The custodian is
de-icing these areas and checking them more frequently throughout the day. Mr.

Purrell also stated that he has submitted a request for the rain gutters to be repaired.
Mr. Purrell stated that a student choked on the bus yesterday from what he heard
was candy. The Medical Emergency Response Team and nurse were called and
were present by the time Mr. Purrell arrived. Mr. Purrell stated that a meeting will
be held to discuss how the incident was handled. Ms. Utz stated that everything
went well according to the report from the health aide.
3.
E. C. Best
It was reported that Mr. Boone is on medical leave and may return to work the first
of February. In the meantime, Amy Weldon is Acting Principal.
4.
Northside
Ms. Utz stated that most of the EpiPens were expiring and new EpiPens were
delivered to every school. There will be a nursing staff meeting on Tuesday to
discuss scheduling EpiPen training for the semester, which must be done by the
second week in February. The spring training will be much shorter, as it will be a
review; there will not be a post test. Everyone will watch the video and use the
trainer again to review anaphylaxis and the appropriate response. The training
should not be more than 20 minutes (15 minutes if the video is watched in
advance). Ms. Utz will contact Numa to set up the training, since there is not a
nurse at that school. The nurses will set up the training at all of the other schools.
Mr. Hogan asked if this was something that the aides could do on February 6th.
Ms. Utz stated that the whole team must attend the training and since she is short a
nurse and is the only person who can conduct the training, holding the training
twice would be difficult.

Churchill County Middle School – Nothing to report.
6.
Churchill County High School – Not present.
7.
Transportation
Mr. Russell reported that the new lifts have been installed. They will start hooking
them up with hydraulic tanks and power, but have to wait until around the 20th of
January. The shop has been working with one lift this winter and it has been very
challenging.
8.
Maintenance
Mr. Byrd reported 400 feet of additional fence slat has been delivered for the south
side of the middle school. Next week, maintenance will add the slats to the fence
up to the old annex building, thereby decreasing the visibility from the houses and
the restaurant across the street. Mr. Byrd stated that this will make the kids a lot
safer.
The board approved new site lighting (LED lights) around the exterior parking lots.
Lahontan and Numa will be completed in the next couple of weeks. The high
school will be done in a month and a half or so.
New blinds were installed in all of the windows at Northside Early Learning Center.
Concrete has been replaced due to a couple of tripping incidents in an effort to
prevent trip hazards. Quite a few fencing improvements have been made as well.

At E.C. Best Elementary, the general contractor will replace a significant amount of
concrete on their dime. The concrete is collecting ice as it was not installed
properly.
The gutters at Numa will be addressed immediately. Oasis will be called to seal up
the leaky joints to decrease the amount of ice.
Mr. Byrd stated that the School Board approved the District-wide intercom upgrade
and he is in the process of obtaining the proper license. Innovative Communication
Systems will begin implementing the intercom changes and upgrades at all sites;
summertime for Lahontan and the middle school, but immediately for NELC, E.C.
Best and Numa.
Ms. Utz asked if this meant that there would be a speaker in the old office and the
nurses office at NELC, as she currently cannot hear what is being said. Mr. Byrd
stated that everything will be linked together with the new intercom. The District
Office will be able to communicate with every classroom at the push of a button
and can immediately put everyone into a lockdown situation. Mr. Byrd stated that
he is really excited about the new intercom system, as there are endless things that
can be done with it. The intercom system will be operational this spring, excluding
the middle school and Lahontan, which will be completed by the end of the
summer.

Old Business
1.
Update on Olweus Training
There was no one present to give an update on Olweus training.
VII.
New Business
1.
For Discussion Only: Workers Compensation Reporting
Mr. Rogne reported an increase in claims: nine new claims (basically unforeseen),
two possible surgeries, and one claim with a possible settlement. Ms. Pruit stated
that the claim is from the previous insurance company, but will still affect the
District’s ratings. Ms. Pruit stated that there are three employees still out on light
duty. Providing light duty has helped immensely. There are no time loss claims.
There were a couple of angry student type issues and training is ongoing with that.
Mr. Morrison asked if anyone from the District accompanies the employee to the
physician. He stated that this is recommended in industrial safety management; for
the employees benefit, as well as for the District. The role the District would play
would be to understand the treatment; is it purely preventative or covering the
physician’s risks as opposed to is it a necessary treatment. The District is paying
Workers’ Compensation. and protecting the employees from being hurt is the
primary goal. Mr. Morrison stated that it is becoming very common in an
industrial setting to send representatives, especially if it is a serious injury. Mr.
Russell stated if it is serious, especially if it is maintenance or transportation, the
employees are escorted for treatment. Mr. Russell stated that if the District sent a

representative every time, we would have to have a contact person in the District
who would at least meet him/her there. Most of the schools take care of their own
and if they have the ability to go with the employee, they should. If they cannot,
some type of procedure would need to be set up. Mr. Russell stated that this is
something that can be looked at, but he would like Dr. Sheldon in on that
conversation.
Mr. Morrison asked how many recordable and restricted work cases and loss work
cases there were last year. Ms. Pruit stated that she has not completed filling out
the OSHA 300 yet, but the numbers are better than 2013. Mr. Morrison asked if
there was a program within the District to educate all of the employees (educators,
non-educators, management) on safety observations; if they see something, i.e.
rolled up carpet, to turn that in as a safety observation. He also asked if it is tracked
in a work order system. Mr. Russell stated that two years ago the District was
doing safety inspections at all sites and staff was involved. Mr. Russell believes
there is a little more reporting going on with the teachers and administrative staff.
Mr. Russell stated that this should probably be approached again and mass e-mails
sent out. Mr. Byrd stated that there are a significant number of safety issues
reported through the work order system, but it would be good to send an all staff email. Mr. Morrison asked if the person who initiated a request is notified once the
concern has been addressed. Mr. Byrd stated that it usually ends with the office
staff or administrator who submitted the work order as staff does not put in work
orders. He was not certain if office administrators communicated back and it is not
part of the protocol.
The committee discussed the Workers’ Compensation investigation process and the
forms related which includes the investigative form and incident report. Ideas for
improving the forms process were also discussed.
Mr. Morrison asked if the District had a form for near misses. Mr. Russell stated
that he does not believe so. Mr. Morrison stated that this takes a lot of energy and
time, but is a good method of forward intervention. Mr. Johnson commented that a
safety box or turning in a site concern has been discussed in the past. Mr. Byrd
stated that there really is no difference between a maintenance work order, safety
work order or security work order, they all get funneled in through the maintenance
work order software program. Mr. Byrd discussed the possibility of expanding the
work order system to work orders maintenance, preventative maintenance, safety
and security types of work orders.
Mr. Russell reported that AIG (insurance carrier) will be here on February 6, 2015,
to perform a site assessment.
VIII. New Business
1. For Discussion Only: Student Visual Reports, camera usage at schools for unsafe
conditions.
Mr. Russell briefed the committee on student visual reports/camera usage at schools
for unsafe conditions. A few students at Churchill County High School were given

disposable cameras and instructed to take pictures of unsafe conditions at the school.
Once the film is developed, pictures will be available. Once completed at the high
school, the middle school and elementary schools will have an opportunity. Mr.
Russell asked that principals be thinking of three or four students who would be tasked
to do this.
Mr. Russell stated that it is a requirement by the State and OSHA that all forklift
operators in the District must be licensed or have taken the class. This will be
reiterated to the high school by the Safety Committee and Dr. Sheldon.
Mr. Morrison asked if OSHA has ever inspected the District and the answer was yes.
Mr. Morrison explained what the Safety Conservation and Training System is and the
benefits of an assessment.
