RESIDENTIAL MAINTENANCE AGREEMENT

on the HVAC system equipment at the service location listed above. DBK agrees to service HVAC
systems ________ times per year. The first maintenance service is to be performed in

______________. Service repairs will be done at a discounted rate on total bill. Normal working hours
are Monday-Friday from 8am-5pm, Emergency on Saturday & Sunday (EXCLUDING HOLIDAYS). If
service is requested anytime other than normal working hours, a discounted service charge of
$____________ will be charged for after hour service. Please review backside of this sheet for a
detailed scope of work included in the yearly maintenance agreement.

This agreement may be canceled by either party without penalty with a (30) thirty day written
notice. The company reserves the right to terminate this agreement for nonpayment of the
agreement price. The purchaser agrees that in case it becomes necessary for the company to
take action to collect the balance of this agreement, whether suit be brought or not, the purchaser
agrees to pay all cost incurred for collections, including reasonable attorney fees and purchaser
hereby waives any claims or other requirements of law to hold Company liable.
