Application Report on the Use of the KLS Martin marSeal
Instrument in Conjunction with the KLS Martin HF Generator

From 29.01.2008 to 07.08.2008, we used in Jena the KLS Martin device
combination consisting of a 10-mm marSeal instrument and the HF
generator maxium® for various gynecological interventions.
Interventions:
The marSeal/maxium® combination just mentioned was used for surgical
interventions in gynecology, especially for salpingectomy, adnexectomy,
hysterectomy, omentectomy and adhesiolysis.
A power setting of 80 W was used on the HF generator, as indicated for
the type of current in question (SealSafe®) and the above-mentioned
instrument.
Assessment:
The reusable 10-mm marSeal instruments used for these indications
were employed for the most diverse sealing tasks involving subsequent
tissue dissection. Thanks to the modularity of the system, which offers
two different shaft lengths (200 and 370 mm), the instruments can be
used in laparoscopic as well as in open surgery. No previous vessel
dissection was necessary in any of these cases.

Compared to disposable instruments, no differences could be found regarding the sealing
results and the time consumed by the sealing process. What must be mentioned
positively in this connection were the minimal lateral heat spread and the low incidence of
tissue adhesion to the contact surfaces. Where tissue adhesion did occur in very rare
cases, the particles could be very easily removed with a moist cloth saturated with saline
solution.
The easy handling during application resulted in shorter operating times, less lateral
damage, fewer intraoperative secondary hemorrhages, and therefore less patient stress.
The lowest Hb-value was 7.6 mmol/l. None of the patients needed a blood transfusion.
As the instrument can be fully taken apart and reassembled with ease, reliable cleaning
and reprocessing is guaranteed at any time.
Conclusion:
The marSeal/maxium® device combination used in the above applications stood the test
in routine use at our hospital.
The essential advantages that deserve mentioning here include the sealing result owing
to a good grasp and firm hold of the tissue, the subsequent dissection without any need to
exchange instruments, the minimal lateral heat spread and the minimal amount of tissue
adhesions. Considering that the tissue structures encountered in hysterectomy make very
high demands not only on the mechanics of the instrument, but on the sealing result as
well, the marSeal instrument can also be recommended for use in other fields, e.g. in
visceral surgery.
Given the resterilizability of the marSeal instrument and the low cost share of the
disposable portion (just the blade), the instrument is extremely attractive from an
economic point of view as well.
The availability of 5-mm instruments and a slight bend in the distal area were topics that
were jointly discussed with the developer.
