Nomination Form for 2011
“
The small effort of submitting the nomination form
made a really significant difference to our hospice, our
volunteers and to our patients. Nominating Carol boosted her
confidence and acknowledged to everyone in the Newcastle
Hospice the great contribution she makes to the team.

How to make a
nomination
To nominate someone you believe
deserves an award simply complete
and return the nomination form
attached, selecting the most appropriate
nomination category.
Nominations are welcome from both
employees and volunteers. Any individual
or group that has volunteered for
Marie Curie Cancer Care is eligible
to be nominated for a Volunteer
Achievement Award.
If you would prefer to submit
your nomination by email an online
nomination form can be downloaded
from the Marie Curie Cancer Care
intranet.
The closing date for receipt of
nominations is the July 15, 2011.
Please think widely and generously
when making your nominations.
The following information should be
noted:
• People can not nominate themselves.
• Volunteers who do not win an award
will not be contacted.
• It is entirely the decision of the
nominator if they wish to inform
the individual/group that they are
nominating them for an award.
• Unless specified as a group or
team category, no more than three
volunteers may be put forward per
nomination. Nominations which exceed
this limit will not be considered.
When making your nomination please
provide us with as much background
information as possible. It is important
that the judges are able to picture the
person you are nominating.
Please feel free to supply additional
information, including photos, with your
nomination.
The judging panel
All applications will be judged on a
national basis in September. The ultimate
decision as to who will receive the
awards will be made by a judging panel
comprised of Marie Curie Cancer Care
volunteers and supporters of the charity.
Awards ceremony
The awards will be presented in December
2011 at a national ceremony in London.
The ceremony provides an opportunity to
gather all the award winners together to
honour their achievement and celebrate
the contribution of all Marie Curie Cancer
Care volunteers.
Judging process
Your nomination will be judged against
three criteria: effort and enthusiasm,
impact and benefit, and exceptional
contribution. When completing your
nomination form it is important that
you try to demonstrate as much of the
following as you can:
Every year Marie Curie Cancer Care
relies on the support of thousands
of volunteers who freely contribute
their time, skills and experience in
a multitude of ways, so that we
can provide care to patients and
families.
Effort and enthusiasm
Volunteers act as charity
ambassadors and help to raise the
vital funds that keep our services
running. They extend the care that
we provide in our hospices and in
the community. They are the high
street face of the charity in our
shops, and offer behind the scenes
support in our offices.
• The individual’s/group’s enthusiasm
in volunteering for the cause
• The ongoing effort made by the
group/individual
Impact/benefit
• How the individual/group has
responded to a specific need
• How their voluntary contribution
has made a difference to
Marie Curie Cancer Care
Exceptional contribution
• That an exceptional contribution has
been made by the individual/group
• Their contribution has provided an
inspiring example for others.
The judging panel may select up to two
winning nominations in each award
category (one in group award categories).
Nominators will be informed of the
decision of the judging panel in
October 2011.
Only with support from volunteers
can Marie Curie Cancer Care
continue to provide more care
for patients and families. We are

and celebrate the extraordinary
support that Marie Curie Cancer
Care receives from volunteers
through these
