2. Differentiate between risk factors, protective factors and warning signs of suicide most
relevant to pharmacies
3. Identify effective means reduction strategies for suicide prevention
4. Demonstrate the ability to develop a comprehensive and collaborative safety plan with
someone at risk for suicide
Objectives for Technicians:
1. Define the scope of the problem of suicide in Utah
2. List risk factors and warning signs of suicide relevant to pharmacies
3. Identify effective means reduction strategies for suicide prevention
4. Create a comprehensive and collaborative safety plan with someone at risk for suicide
Conference Details
Registration: Registration fees are discounted before March 28th, 2017. To register, complete the
registration form and pay by credit card (online only) or send a check to: USHP, PO Box 58356, Salt Lake
City, Utah 84158. Space is limited so please register early! Registration fee includes admittance to all
programming, exhibits, and group meal functions. Registrations received after the March 28th deadline
or on-site are subject to an additional $25 processing fee.
CE Credit: To obtain up to 4 credits of continuing pharmacy education credit (0.4 CEUs), the participant
must attend the conference, check in at the registration desk, complete and turn in the attendance and
evaluation forms, and participate in the active learning activities. Attendance forms not turned in on
site will not be accepted for credit. A Statement of Credit will be available to the participants within 60
days of the program.
Cancellation Policy: To receive partial or full refund, registrants must contact USHP in writing prior to
March 28, 2017. Requests made after March 28th will not be considered.
Exhibits: The exhibit area offers an opportunity to learn about the latest from the pharmaceutical
industry. Representatives from pharmaceutical manufacturers, wholesalers and others may be on hand
to demonstrate new products and services.
Special Assistance: It is USHP’s policy to comply with the American’s with Disabilities Act. If you have
any special needs or services while attending, please contact USHP no later than March 28, 2017 at

Parking: Parking is complimentary and is located adjacent to the building. Directions to the facility will
be available on the USHP website (www.ushp.org) prior to the conference.
