and the team. Each Memory Mate is customized with the Team/Group Name
or logo and the year. It comes packaged on a cardboard mount with an easel back.
Ceramic Mug – A 15 oz. ceramic mug featuring your child’s picture, name

Dog Tag – Single sided oval tag. Includes a 24” chain.
Flat Magnet – BEST SELLER! 3x5 flat refrigerator magnet. Includes child’s
name and year.
Fun Pak – Includes one ruler, two bookmarks, one doorhanger, three key tags

Little People Magnet – A great addition to your refrigerator! A 4x5 cut-out image of your child adhered to masonite with a
magnetic strip on back. Please Note: This is a specialty item and will take approximately 2-3 weeks longer to
produce than the rest of your order. This item will be mailed to the address that appears on the front of this order form.
Magazine Cover – Showcase your child on the cover of a magazine!
Specialty Keychain – Metal keychain that is sport specific; shapes include baseball, softball, basketball, volleyball, football
and megaphone. Other sports use round shape with photo on one side and custom design on other side. Circle keychains
are 2½ diameter. Oval keychains are 2½x1½. Megaphone keychains are 3x2.
Statuette – An 8x10 cut-out image of your child that stands upright on a base. Please Note: This is a specialty item and
will take approximately 2-3 weeks longer to produce than the rest of your order. This item will be mailed to the address that
appears on the front of this order form.
Trader Cards* – Very cool! Your child’s picture, name and year will be displayed on the front of the card. The back includes
all the important information that you’ll want to remember for years to come. Please be sure to fill out all the player
information shown on the front of the form.
Wood Plaque – A 4x5 individual photo and a 5x7 team photo mounted on a 9x12 wood plaque. Includes a custom
imprinted metal plate with team name, logo and year.
