GRANT APPLICATION
Your application must be submitted to [email protected] and accompanied by three required documents:
1. Permanent IRS determination letter with federal designation # as a 501c3 charitable organization
2. An original letter with your grant request, directed to Ms. Doris Day, Grant Committee, Doris Day Animal
Foundation. This letter must be on your group’s letterhead, and signed by an authorized member of your
organization.
3. The organization’s most recent IRS Form 990 or audited financial statement
The Doris Day Animal Foundation only rarely funds any type of capital expansion, land purchase or large equipment
acquisition. Most grants range between $1,000 and $5,000, and we try to tie grants to helping animals as directly as
possible.

What is your organization’s purpose or mission statement?
Please list your board members and length of their service related to animal welfare causes.
How many staff members and volunteers does your organization include?
Do you contract with any local governing bodies?
How many animals (on average) are in your care each month and each year?
Please share any statistics on adoptions, fosters, or long-term animal residents in your organization. Add any detail
about what your group does and how well you do it.
What city or region is your work concentrated in?
Where do the animals you help come from (strays, owner surrenders, city animal control)?
Do you have a shelter facility? Or do you rely on volunteers caring for animals in their homes? Please attach photos of
your facilities, if available.
Why is your organization in need at this time? If awarded, specifically how will the grant funds be used to help animals
directly? (The more detail you can provide about the nature of your need – whether it’s for emergency veterinary care,
spay/neuter, food/shelter, transportation, pet pantry, how many animals will be helped -- the better your chances of
receiving an award.)
Will the funds be used in an ongoing program?
What are your other funding sources?
Describe briefly the details and results of specific projects – start/stop date, individuals involved, events that may occur,
etc.
Please quantify your financial needs with specifics of what it costs to fund a certain component of a program, or how
much each animal’s care costs for a certain period of time. For example: A DDAF grant could help care for x animals at x
cost for x months, or provide x animals with vetting needs prior to rescue.
Does your organization care for a senior animal population? If so, please tell us about your specific senior rescues or
programming.
What is the average annual intake and adoptions of senior companion animals, and what emergent needs do you
encounter with these animals?
HELP US LEARN MORE ABOUT YOUR ORGANIZATION:
Please attach (scanned) letters of reference from adopters, volunteers, veterinarians, local businesses – anyone who can
help us understand how your organization is helping your community and the local animals in need.
Also include photographs of your facilities or foster homes. These will help us reach a decision.
Please scan and attach any new clips, awards, brochures or other print materials about your organization that show you
in your best light.

NOTES:
A report from your organization will be due once a DDAF grant funded is expended. If project is ongoing, you will be
required to file a report once every 6 months. Your report must include photos and stories about the animals the DDAF
grant has helped. Copies of veterinary invoices, where available. Reports should be in digital format wherever possible
and emailed to [email protected]
No services to DDAF will be provided as part of or in conjunction with any donation.
