Instructions on filling out FAA form 8610-2 1. You may fill out the form by hand in black ink OR save the form to your desktop and fill in via computer 2. You will need 3 original copies, all filled out to bring to you interview (don’t make copies) 3. Note that the 8610-2 has 3 pages. Only fill out the second page, but print out the third page as well. The first page is Privacy Act info, read and discard. 4. At top of form, check boxes for “mechanic” “airframe” and “powerplant”, and “original issuance”. (Regardless of your experience, check all 3!) 5. BLOCK I APPLICANT INFORMATION: Fill out completely. Special notes: a. NAME: COMPLETE LEGAL NAME. FIRST, MIDDLE, LAST b. Social Security block: write in “DO NOT USE” c. DOB: example: 08/03/1979 d. HEIGHT: IN INCHES ONLY do not write feet and inches. If you are 6 feet tall, write in “72” e. Nationality: USA f. Place of birth: Give: City, State, Country 6. BLOCK II CERTIFICATE OR RATING APPLIED FOR ON BASIS OF: a. Check only the block “Military Experience” DO NOT WRITE ANYTHING ELSE IN THIS BLOCK! 7. BLOCK III RECORD OF EXPERIENCE: Fill out completely. Special notes: a. Fill out “Military Competence obtained in” blocks 1,2 and 3 b. Fill out dates from/ to, employer and location and type work performed. You will need to type or print VERY small, the FAA wants as much information crammed in as possible! c. EXAMPLE: d. Dates: From 08/1995 TO 09/2001 e. Employer and Location: USMC, VMA 223, MCAS Cherry Point, NC f. Type work performed: AV-8B inspection and repair. Performed daily inspections, serviced hydraulic, fuel, and oil systems. Troubleshoot discrepancies. Perform engine borescope. Conduct engine power check. 8. BLOCK IV APPLICANT’S CERTIFICATION: a.
